Choka is not the bio daughter of Tsurugi Shiranui nor did she get her scars from a car incident. The truth is much much worse
Her real name is Oiwa Shigaraki, she was one of the many children that All for One have where over the 100 years roughly, he has been having children through manipulating women and intentionally sabotaging birth controls to get them pregnant secretly where he stays with them and convinces them to either keep them or if not, he’ll happily take care of the baby by his own.
When the babies are born, he uses one of his quirks to erase their memories of their own children once they’re home and kidnaps them so that he can turn them into his weapons against the hero society and planning an potential successor. He would allow Garaki to experiment on them.
But many of them brutally died from the AFO quirk experiment, so when AFO found Tomura, he stopped that specific experiment for his children but continued the others where Garaki makes them super assassins.
Oiwa/Choka is one of those children where she had her first kill at five to prove herself. She was brutally experimented where she felt the needles injecting poisons into her just to get her to become poison resistant since she was born quirkless till at 7, AFO gave her the Poison Blade quirk once her poison resistance is high enough to handle it.
She was also terribly isolated from the outside world along with her siblings till she became 11 when she begun her assassination and even then, she is restricted due to the tracker in their bodies. For her, it’s in her brain so if she tried to escape, it would automatically blow her brains out.
After years of watching her many siblings get killed by their father through either failing too many times through either drowning, burn alive, strangled to death or worse, get forcibly turned into a nomu or eaten by one. She was forced to watch all of this as a warning along with the other siblings she had remaining.
By the time she’s 13, she already has the double digit kill count and she was assigned to kill Tsurugi Shiranui, the 11th pro hero (OC) where she was gonna kill her in her sleep just like she had with her previous victims but Tsurugi woke up and it lead to a fight.
When Oiwa/Choka loses, she internally freaked out and, instead of running to her father like she meant to, she runs to the roof which Tsurugi tried to stop her along with other pro heroes and Naomasa but it’s too late as she already jump out of the roof to end it all for failing her mission.
She wasn’t saved on time, thus she ended up hitting her head in her fall, which lead to an emergency surgery which unintentionally the fall broke the tracker in her brain that Garaki installed to ensure none could escape. That’s how she truly lost her memories
When she had her blood tested, they realised she’s a DNA match with AFO which lead to an emergency meeting with the pro heroes and hero commission over what to do. The commission were planning on taking Oiwa in and secretly trained her like they had with Hawks, but Tsurugi volunteered to take her in which concerned the heroes, though she refused to back down and All Might supported her decision as a old friend.
So, when Tsurugi visited her when Oiwa woke up, she admits she can’t remember a single thing, not even her own name. So, Tsurugi, noticing the flowers and a butterfly on it, “reminded” her that her name is Choka and she is her mother where she explained she was in a terrible car crash. Thus, that’s how the lie of Choka Shiranui was born
