What’s the difference between Orthodox Jews who have peyos and who don’t?
All Orthodox Jewish men actually have peyos - it’s forbidden to remove the hair from that area. So what you’re actually asking is what the difference is between the ones who have long, obvious peyos and the ones who have more subtle ones.
It’s a matter of custom between different groups and even different families. Here’s at article from chabad.org that elaborates a bit.
One of the commonest classes of religious objects found in Late Bronze levels is constituted by the so-called "Astarte" plaques. These are pottery plaques, generally oval in shape, on which were impressed (from a pottery or metal mould) a figure of the nude Goddess Asherah, en face with her arms upraised, grasping lily stalks or serpents, or both, in her hands. The Goddess's head is adorned with two long spiral ringlets identical with the Egyptian Hathor ringlets. These plaques were borrowed from Mesopotamia, where they have a long prehistory in the Early Bronze Age [about 3200-2100 BCE].

Let me tell you about this boy’s peyos. My husband doesn’t wear his long. But the rabbaim do & he is in playgroup with kids who have them.
So before his upsherin he started talking about how he was going to have peyos and of course we didn’t want to discourage so we just haven’t touched them.
This is what they look like before and after I make them for him.
He says “thank you mommy for making my peyos round!”
Three tired Hasidim, peyos askew, smoke cigs bought from Pakistanis.
Omer Problems
I have peyos now. The right one is longer than the left one.
