Bart Simpson was based on a real person who had severe jaundice, and a real condition that caused hard, spiky growths to form on his head. He wore a hat to cover the growths, but eventually they grew into and interwoven with his hat so he couldn't take it off without scalping himself.
He died of severe bloodloss after seeing a lady about to cross a puddle and he reflexively took his hat off and laid it down for her to use as a stepping stone.
