In canon 5.21, he wakes up in a hospital and explains to Dean that he was found on a shrimping vessel off Delacroix, Louisiana, scaring the fishermen.
I always wondered what might have happened if Cas had instead crash-landed on the ship of a fisherman who was actually an undercover/retired Vampirate.
So now, thanks to the Benny Bang, you get to see this cracky concept come to fruition. Benny tags along with Castiel to save the world, and maybe falls for a quirky little fallen angel in the process.
AU where after a mishap breaks the earpiece, Chloe is mayor without any of Lila’s instructions. To everyone’s surprise (including her own) she’s actually doing a more than decent job. She eventually throws away the earpiece figuring she doesn’t need Lila to tell her what to do anymore. And for the first time since she was Queen Bee, she’s looking after the city. She can even tell people who she is this time.

She knows he’s switched from Clarke’s Bellamy to Blodreina’s Knight. Rooting himself in their language so he can carry out their ways.
Part of him would still be Bellamy, but part of him would never be the same again.

Ya see now I'm picturing Fem!Sam and Dean having so much fun with this new body. The curse wears off after a few weeks. They wake up to Sam back in his regular body and they just keep the new relationship going. Jokes here and there how it was easier as a girl because Sam's body self lubricated. But now it's been about 2 months since then, Sam feels sick and they both think it's a cold/flu/stomach bug/anything because of they are constantly on the road.
Let's say this is set during season 3 because I'm a dramatic person and they get a call from Missouri. Firstly getting after Dean for the deal but understanding why and feeling sorry for them. Secondly because she can't believe Sam's pregnant and that the curse isn't fully gone.
See when Sam got whammied by the curse, it had certain conditions. Fertility conditions. Now Sam will always be able to carry for him and Dean.

S3 pregnancy could make Dean come around sooner to trying to save his soul from the deal. It puts an even angstier spin on the Doc Benton ep if they still go there. ("So, what, we're going to raise our child as zombies? What the hell do you think that's gonna do to a kid?!")
Omg, but imagine Dean still goes to Hell and comes back s4 and finds out RUBY'S been helping Sam raise their baby. Oh lord.
(Quick aside: if I imagine Sam actually can and does breastfeed, how does imbibing demon blood affect things? Does he refuse demon blood for his child's sake? Or switch to formula and do demon blood anyway? Probably switch. But imagine if Ruby convinced him he didn't need to switch. Imagine Sam and Dean's baby going black eyed and outing Sam's extracurriculars 😱😱😱)
But before he found that out, would Dean be devastated that his brother followed in their father's footsteps--hunting with their child in tow, obsessed with getting revenge for Dean's death? Except no one's seen Sam for ages, so who's been helping Sam out the way all Dad's friends helped him out with babysitting?
Cue the Ruby reveal. Cue breakup arc with a fucking custody battle. Cue Dean getting verbally smacked down by Ellen and Jo for sexism when he goes to them first for baby duty. Oh brain, why?! 😭

Imagine Sam gets pregnant again and in that confrontation with Zachariah, instead of taking Sam's lungs he takes Sam's entire womb.
Maybe he does both.
Maybe he leaves the womb but Sam's bleeding out between his legs.
Imagine Sam and Dean having to entrust their friends with babysitting shifts in the panic room to keep their child safe from angelic or demonic kidnappers.
But wait, let's rewind.
Let's say Missouri does call them with the surprising news, and also has information to help them break Dean's deal. Somehow they get Dean out of the deal and they both get to live. Sam works with Missouri to strengthen his psychic powers and keep his family safe.
But this results in more boyking!Sam cultists coming around for them. Sam's ability to get pregnant is some kind of sign of his coming, and there are subsets that try to kidnap their baby for worship. There are also Lucifer followers still trying to fulfill their plan, wanting to kidnap the baby for leverage or sacrifice.
Sam and Dean always manage to defeat them, but it feels like they can never rest. When Bobby builds the panic room, he suggests they leave their daughter (ok, I'm deciding they have a daughter. She's almost 2!) in his care for a weekend and recuperate/have a honeymoon.
It's when they find out that Sam's pregnant again that the angels make themselves known.
I'm starting to wonder how S4-5 would have played out with Lex if Lena'd had Paladin. Presumably, Lena would still work with Lex but probably only because he knows her secret and has an LRAD or something to coerce her into helping him. Especially the second time in S5. Because first of all Paladin would not be down for mind control and honestly the thought wouldn't even occur to Lena because in this version she's still in character, so Non Nocere isn't a thing unless it's Lex's thing. Which *would* be in character for him.
Which means that a) even after the rift Lena wouldn't have been alone, and b) any time Lex gets done talking with Lena, Paladin hisses a different dipping sauce every time Lex leaves the room, as a reminder that they'll eat him, eventually.
