I originally wanted to give pony Lester rain rot (sounds worse than it is, look it up) which is a skin condition in horses that comes closer as a parallel to what acne can be like, especially if inflamed, but I couldn't make it look right, so he has vitiligo instead now, which is a purely cosmetic condition.
