(Backstory is based off the film,I didn't see the serie)
He probably would have realized he's trans in his late teenage years. Definitely only after he moved in with his aunt. Before he had been too young to know,and then he had to constantly be in survival mode and he was dealing with intense trauma,so he really had no chance of realizing that.
His aunt would definitely be supportive and help him trought this journey. I really can't see her being transphobic towards the only person she has left.
His transition is quite literally perfect. He start hormomes fairly soon despite the times thanks to his connection to medicine. His aunt financially helps him trought it so that he only gets the best equipment. He is extremely good at voice training too. In barely a few months,he has mastered a perfect,natural masculine voice and masculine manners. Even people who knows he wasn't born a guy sometimes forget about it. He perfectly blends in,without losing any of his class.
As he grows up,his transition keeps going very well,and he mostly keeps it secret. Not in a jelaous and definitely not in an ashamed way. It's a very personal information about him and his past,so of course he won't just tell anyone about it.
Almost no one knows that doctor Hannibal Lecter wasn't always a man,and he keeps it that way.
Also I want to point out that in this AU,he chooses his name. And I can absolutely immagine him going 'you know what would be very amusing?' And settling for Hannibal. Because of 'Hannibal' and 'cannibal' (definitely ooc but honestly who cares? It's fanfiction after all).
He probably gets only top surgery done,because breasts are way more difficult to hide,and even the best,most expensive binders restrain movements quite a bit,which is a really big inconvenience for a killer. The scars would only be on his chest,easy to hide from everyone. About bottom surgery,the scar on his arm would be far more problematic to hide and to explain if someone saw it,and he knows perfectly well that his genitals do not dictate who he is,so he doesn't mind having a vagina.
I don't think he would ever feel very disphoric. As I said,he knows that genitals don't dictate one's gender,and even while having a period he remains calm and composed.

He doesn't give much importance to his partner's gender,he's more attracted to one's person,their soul if you will.
Will most definitely help you transition if you don't know where to start,or don't have the resources to. He's rich,he has connections and knows perfectly well the best way to go trought this journey.
He doesn't particularly care if you don't want to medically transition. He just makes sure that you know that if you want to,you could do it without any problems with his help. And if you don't,that's your choice. He also doesn't mind if you prefer dressing or acting in a more stereotipically feminine way. He's very aware that none of it changes who you are. In his eyes,whatever you do,you will always be as much as a man as he is,at least in terms of gender.
If you take testosterone,he aligns his schedule with yours,so that you both have the same shot day. This is both so he can help you,since he's very skilled with needles,and because he knows that some of the side effects of testosterone are much more enjoyable in company,if you know what I mean.
If you're feeling dysphoric,he usually prefers to let you process your emotions,gently guiding you trought it and supporting you. Homewver,he could probably manipulate you into feeling better if he deemed it necessary. He doesn't usually do so,but it's always an option.
If you still get your period,just know that he knows. He knows your cycle better than you do,and can always tell when it's coming even if you have irregular menstruation. He has all the products you might need and is ready to accomodate any of your needs and cravings,as long as they're resonable of course. He can cook about anything you might want,and if you're particularly sensitive to strong smells or tastes during that time of the month,he can make something more bland. If you happen to stain the sheets,he knows how to wash away the blood stains. If you're moody or angry,he somehow knows exactly how to calm you down and comfort you. And if you want to medically stop your periods forever,he is more than ready to help you go trought the sistem and operation.
He will not tolerate any disrespect towards you or your identity,from anyone. Any insult to you is a direct insult to him,and he does not accept rudeness. The transphobe's corpse won't even be used as food. He would never serve such a lowly creature to the love of his life
