Their first time, as i imagine it.
It all starts with the episode Dream a Little Dream of Me. When sam enters dean's dreams and sees, well, something. Something that makes him realise dean wants him in a way he would have never imagined.
He is quite shaken and disgusted at first, doesn't know whether to punch his brother in the face or bury the image deep in his mind and never think of it again. But before he can make up his mind, dean makes a confession. Not that he's secretly in love with his little brother and wants to fuck him senseless, having wanted that since he was fucking fourteen, no. But something similarly disturbing: he admits to being terrified to die and go to hell.
Dean being vulnerable literally never happens, and while sam already knows that dean is very much scared under his mask of macho nonchalance, he only now understands just how terrified dean actually is. He can't bring himself to be really upset with him after that, but can't stop thinking about what he had seen in that dream.
In Mystery Spot, after dean dies for the hundredth last time and all sam can do is spend the next six months obsessing over and tracking down the trickster not caring about anyone and anything else, he learns something about himself. He learns he's willing to do anything for dean.
This realisation, while not entirely unexpected, somehow makes perfect sense. It plants a tiny seed in his head. He spends every waking hour after getting dean back contemplating everything. He fosters that little seed, keeps it warm and moist, and soon it develops into a fragile little plant. By the time dean's time is almost up, the fragile little plant grows into a stubborn weed, one that cannot ever be removed. Sam made a decision, one that will change the trajectory of his life forever. One, that while changes everything, really changes nothing at all.
He decides to give dean something he knows he really wants. Himself.
He approaches it like any other job; by doing research. He spends his free time studying gay porn, reading about how to please a man, how to prepare. And when the time comes, he is prepared. He has a plan, and he's determined to go through with it. He's ready to give dean everything he wants.
