Bruce once loved Tommy Elliot like a brother. Their families seemed close, and for all the bad in Gotham, Bruce thought that he at least had a friend in Thomas. They drifted apart as they aged, Bruce became invested in a need for justice, and eventually left Gotham entirely. When he returned things were never the same, he had no idea that Thomas had come to blame him for so many wrongs he had suffered, nor that the man envied his life so thoroughly.
Hush crossed a line that split any remaining vestiges of their friendship. Posing as Jason Todd (who Bruce was unaware was actually alive at the time) infuriated Bruce. Not only had hush dredged up old memories, painful memories, but he also threatened Tim Drakes life. After losing one Robin, Batman to threats like that very seriously.
He's also more than a little off-put by Thomas' self-surgery. Seeing a scarred mockery of his own face whenever he looks at Elliot is difficult. He still remembers the boy from his childhood, but he looks nothing like the man Bruce knows today. Elliot takes everything that Bruce stands for and distorts it- all while wearing his face.

Oh my god no I don't think you realize the staggering amount of sheer loathing Harley has for Thomas Elliot. He managed to get Joker on-board with his first plot to kill Batman, but when Harley tried to kill Batman-- to make Joker happy, no less, not out of personal vengeance— Joker told her nobody was allowed to kill Batman except for him and then pushed her out a window. She hates Tommy so much for it, even if it wasn't necessarily his fault. Not only that, but he was engaged to Peyton Riley, and even if he hadn’t gotten off scott-free where Harley had nearly died for trying to kill Batman, she would have hated him just for being so closely associated to Peyton. Harley sees them both as cowardly, shallow people who pervert and destroy The Way Things Are to serve their own selfish needs. He waltzed into her world and fucked with the fabric of her reality, and wasn't even punished for it. There are no words in the English language to describe the sheer amount of burning rage this man inspires in Harley.
