Consider this: an AU where the rule that medicine cats can’t have a mate or kits because it might distract them from their duties is taken to its logical extreme- they can have no relationships at all, outside of other medicine cats. Medicine cats are chosen by StarClan when they’re only a few days old. The current medicine cat then takes the kit and raises them in the medicine den, just outside of camp. Telling the kit about their family is forbidden and harshly punished, as it’s seen as “tainting” them- medicine kits aren’t even allowed to go into camp until they’re around three moons of age, which is when they’re old enough to understand how important it is that they maintain a level of aloofness around other cats. Medicine cats are also banned from making friends with cats in their Clan, only ever talking to them for healing matters. If medicine cats had friends or family, it’s feared they would be biased in healing and in prophecy interpretation, and that they would lose StarClan’s favor. This makes what Leafpool did a hundred times worse in the eyes of the Clans. It also makes it a lot harder for Jayfeather, who realizes that if he’s ever going to figure out who the other cats “with the power of the stars in their paws” are, he’s going to have to figure out who his kin is.
