Therapy can't help Bruce Wayne because he is determined to follow this path. He trusts no one but himself and he trains Robins because they're the only people he can trust (only partly because they're the people he can control.)

Below is another panel from Heroes in Crisis. Near the start of the storyline he promised Diana and Clark that therapy is helping and he trusts them. Crucially, he promises that he no longer carries Kryptonite "just in case." Then he's lassoed by the Lasso of Truth and:

He can't trust even Superman or Wonder Woman. He will do a good job of seeming to change but he can't. The needs of the comic book form have trapped him forever and any moment of happiness he gets is transitory. At best a warm memory at worst foreshadowing of even worse loss. He's a tragic hero and he's worse than a tragic hero because he can't rest even in death.
