I want to talk about how "self-harm" and "self-medication" are subjective designations, frequently applied to the same set of behaviors, and whether you choose one or the other often depends on whether you see the self-harmer/self-medicator as a human being with moral agency and legitimate decision-making skills, or as a crazy person who needs to be fixed
I used to cut myself regularly because I was unbearably depressed and cutting gave me these little moments, like less than a second per cut, where all I felt was the pain and the rush of adrenaline from the minor, superficial injury. For that half-second, I didn't feel depressed. I could forget about how much I hated my life. I just wanted a break, just for the briefest instant. I wasn't really hurting myself in any meaningful or lasting way. I would clean and bandage the cuts when I was done. And it helped. It made me feel better. It really, really did. But people have always been much more freaked out by the fact that I was cutting myself than they were by the fact that I was suffering so much, constantly, in my day-to-day life. People say cutting is bad. I don't think it is. I think it just makes people who don't self-medicate in socially-unacceptable ways uncomfortable. That's all.
I've gotten 2 courses of ECT now. My depression is as bad as it's ever been, and the side effects have been significant and seem to be permanent. I've quite literally forgotten entire years of my life as a result of the ECT. All those memories, gone. Everything completely erased. I have little snatches here and there, things that I know happened, but cannot actually recall. It's a tremendous loss. I mourn it constantly. And in return for those memories, I have gained nothing but medical debt. Cutting helped me feel better in the short term. ECT didn't help me feel better at all. One of them is stigmatized, shocking, scary, unacceptable, and one is not. One of them left me with minor scars on my skin, and one of them left me with no visible marks and a gaping void where my past should be. Let me be clear -- I'm not saying that the physical visibility of cutting is the entire issue here. What I am saying is that the difference between what we call "harm" and what we call "treatment" is artificial. It's cultural. It's medicalized. It's prejudiced. And it's a fucking problem.
I used to get pass-out drunk every single night because when I tried to lie down and go to sleep, I would be bombarded by constant, vivid flashbacks. If I did manage to get to sleep, I would have horrible nightmares -- they're difficult to describe to anyone without PTSD. It wasn't like "oh I had a bad dream" -- they felt utterly, physically, viscerally real. It was intolerable. I didn't have a PTSD diagnosis. I didn't even realize I had PTSD for awhile there. I wasn't receiving medical treatment, but I needed to fucking sleep. So, I drank. It worked. I slept. I'm not saying I wasn't an addict. I'm not saying I don't still struggle with drinking too much. What I am saying is that addicts deserve a little goddamn respect with regards to our substance use. I made a deliberate, measured choice to drink heavily on a daily basis because I needed to sleep at night, but most people expect me to articulate that decision as "I am powerless over alcohol and it has ruined my life," which has never been true, and then tell me I'm in denial when I say so. Just because something is socially unacceptable or bad for your liver or whatever doesn't mean people aren't doing it on purpose because they prefer it to the available alternatives. I am a human being. I am capable of making my own decisions. Just because you (general you) don't like the decisions I'm making does not mean that I am utterly divorced from reality and incapable of caring for myself in any way. You don't fucking own me.
I think we should keep talking about my drinking, actually. I think we should talk about what happened after I got a PTSD diagnosis and got on medication that alleviated the worst of the nightmares and flashbacks and allowed me to sleep without being wasted. "So mar, did you stop drinking then?" seems like the obvious question. And the answer is no, I fucking didn't. Partly because I think (and have always thought) that getting intoxicated is a fun recreational activity, and I don't particularly want to go straightedge, but mostly because my life still sucked shit. When you are drunk, you are not fully present. The booze is a buffer between you and the stark reality of your life. Even when you're totally fucking shitfaced, sobbing on your bathroom floor at 2am and hugging your toilet like a teddy bear, absolutely inarticulate with misery, you're not all there. It's a break from the inescapable, sober world -- it's a way to forget about it for a little while. It's a way to quickly change how you feel. It's a temporary relief. Just like cutting was for me when I was younger.
Given the long-term physical effects of alcohol abuse, I've always found it kind of funny how much more socially acceptable heavy drinking is than cutting yourself. All in the name of good health, of course. Wouldn't want to look crazy now, would we?
