Oh, God, now I remember reading a theory that Sherlock’s mind, to make it easier for Sherlock to deal with it, substituted someone with redbeard. So it wasn’t a dog who died, but a person! And Sherlock started to take drugs after that person died!

What if it’s “the other one” that Mycroft mentioned at the end of HLV? Sherlock lost a brother that’s never been mentioned. Something like that is way more likely to have set him down a destructive path than losing a dog. I always thought the whole dog thing was weird? That’s not nearly dramatic enough of a reason. There’s no way the dog was literal. And, this might be a stretch, but what if it just wasn’t a brother but a twin? A lot of people are saying Moriarty had a twin. And in the special, Sherlock was adamant that “it’s never twins!” Idk. I think it would be a cool twist that he was the one with the twin.

It could be applied to both that theory AND possibly the Moriarty / Janine theory. In this case: “It could have been planned” may refer to the fact it is a known genetic condition to the family, and “since the moment of conception” = born with it. If Mycroft or Sherlock and The Other One were twins, then both possibly have the same thing, something ‘planned’ since conception.
Also: possibly ‘twin’ meaning two, but not necessarily the same, so TWO Holmes siblings have the “planned since conception” thing.
Sherlock denying “it’s never twins!” is him denying that this thing exists in himself and his siblings; doesn’t want to face it.
