No matter Fenrir's opinions of her husband or decisions she makes, Narcissa is part of Regulus' pack, so she's part of his too. And as such, the offer is sincere.
“Well…” Narcissa mused thoughtfully. “If rumors are to be believed, maybe he would like to join his loved in that state… alright. Why not…” she raised her wand a bit. “But let me be clear Greyback, those teeth of yours touch my children, husband, or cousin, and you’ll pay dearly.”
She still didn’t trust him completely, but, she was willing to work with him for Reggy’s sake.
"Well...if he'd enjoy it, it ain't payback," he pondered, "But maybe death'd suit him better. Or I could at least make sure everyone knows about it."
Fierce, he liked it. A lot of Death Eaters lacked spine, which made them boring. The most fun he got was making them squirm. But Regulus, and now his cousin, talk back. Much better for conversation.
