Pharmacists receive no training in animal pharmacology, but often veterinarians must rely on them to fill medications for pet owners - the owner may request to take a prescription to a pharmacy for financial reasons, or the medication may not be available direct from the veterinarian.  Trouble can occur if the veterinarian’s instructions are not clear (some of our pharmaceutical jargon is not used by human pharmacists) or if the pharmacist assumes that the dosages for animals are similar to those for humans.
So far I have had a few cases where a human pharmacist tried to change my patient’s prescription without consulting me; I only found out through the client. Â
In one, I prescribed 0.8 milligram soloxine (thyroid medication) to a 110-lb dog.  Humans take very small doses of thyroid medication, such as 0.01mg.  The owner wanted to try to get it at her regular pharmacy, and the pharmacy tried to send her with 0.1mg tablets, the largest size they carry.  I had to speak to the pharmacist twice before they realized that yes, dogs are dosed very differently than humans.  The dog would have needed to gulp down 8 pills twice a day to get the correct dosage!
In another, I was trying to regulate a newly diabetic cat on glargine (Lantus) insuline.  Glargine in cats can sometimes cause complete remission of diabetes, the only insulin to do so reliably.  So of course I wanted to see if I could get my patient into remission.  Imagine my anger when the client called me saying that the pharmacist was trying to have her take a cheaper, generic insulin — a NPH insulin, which in cats DOES NOT cause remission!  Not only that, changing doses between insulin types is very dangerous, and can cause over or underdosage!  I had to explain to the client that ONLY that insulin had a shot of causing remission, and had to instruct her to not take no for an answer if the pharmacist didn’t want to fill her glargine.
I’m hoping that cases like these will lead to more awareness among pharmacists that animals are not like people, and that our doses are sometimes much higher or lower than they would expect.  I wouldn’t mind the pharmacist themselves calling me to verify — but I do get upset when they don’t even try to check in with me but change things themselves.  Maybe someday they will all keep a copy of Plumb’s formulary in stock…

Yung nag Ha-halitosis (Bad breath) ka dahil sa kape at mani na kinakain ko gabi gabi para maextend yung hours na gising ako. Yung hindi ka pa nga Grumagraduate eh kailangan mo ng magreview for the board exam kahit na wala pang kasiguraduhan kung gagraduate ka ba o it will end up ba in a good way? lahat ng yan pumasok sa isip ko bago pa ko magboard exam. It came to a point na ayoko muna mag take dahil natatakot ako na baka bumagsak? na baka mapahiya ako sa mga tao, Isa pa dahil sa Peer pressure kaya naisip ko bigla na wag na muna. Pero there was a friend na destined siguro na magkausap kami. Siya yung nag-push talaga sakin to take the Risk and to change plans. Siguro kung di kami nagkausap nito baka until now stressed parin ako at sobrang nagsisisi. Pero dahil magaling si God He used this person at hindi nya hinayaan yung desisyon ko na walang kasiguraduhan yung masunod. Look at me now Papa God, RPh na ko! :)
Basta nung after lumabas nung results eto lang talaga nasabi ko.Â
 Hahabaan ko pa sana. kaso tinatamad nako. hihihi. It's already 2:12 in the morning na kasi.
