In all honestly, it's all pretty much CC work. I tend to test with loads of things to increase the realistic look that my Sims have, and I will put a full list of recommendation below (because nobody trying to gatekeep).
First thing first is ofc good skin, @esotheria-sims made an incredible list which I use too, so I will just link the stuff for you to see here. I pretty much use everything on there.
Next thing is sliders. I mean every single slider in existence. I use them all, and test them like crazy. There is a tumblr page called @sims-2-sliders, and you can find them there. Go crazy go stupid with it until you find the best results.
Now the bits and pieces that are so critical for that realistic look.
I cannot stress enough how important is eye sclera for that realistic touch. @digisims2 has great great stuff, linking them here and here. And also there are some old CC that I still use such as Anva sclera . @pooklet ones that you can find here.
Next great thing is eye masks. I think personally its the best invention ever.

Eyes are done, next comes the face. When it comes to it there is soooo much to choose from that I barely can but I will try to be as useful as possible here.
