It's been weeks since Cas was taken by the Empty.
Dean frequently casts a summoning spell designed specifically for Cas, to pull him out of there, but it's not strong enough. Each time nothing happens.
Sam knows it'll never work, but he understands that Dean needs to feel like he's doing something, so he leaves him to it.
But Sam doesn't know that Dean is tweaking the spell a little bit each time. He's using different ingredients, altering the quantities, adding blood, he's doing all the things that someone should Never Do with summoning spells.
But standing where his Cas should be is the Castiel that's filled to the brim with purgatory souls. He's practically buzzing with power, barely containing it, and he looks at Dean with both disdain and intrigue.
Godstiel explains that he can feel every single time Dean tries another summoning spell - across time and space - and it's getting on his nerves.
(Him being able to be here makes no sense, but the god-like powers of all those souls have made him slip out of the timeline, so he exists even when he doesn't. Time is complicated.)
Dean thinks maybe he should be scared, but finally! here's someone powerful enough to bust his Cas out of the Empty! and has no morals about interfering and 'upsetting the natural order'! And besides, it's Cas. He's vaguely homicidal and going insane with power, but it's still Cas.
Dean just needs to convince him to help with breaking his Cas out, which is easier said than done when this soul-powered Cas doesn't like to share.
