Personally, I'm not particularly opposed to the idea of the Red Man (both as a figment of Mona's unhealthy imagination and as a real being). At least the bits of lore we have (machine, the statues, his appearance) look nice and interesting. I wish they'd been given enough attention, and not just thrown in for the sake of it...
In my AU (post-finale), the Red Man was created/summoned by people belonging to high society. Why? I'm not sure myself, perhaps as an exotic pastime (Hey guys, let's summon/create the embodiment of cruelty!), perhaps as an attempt to create a living weapon of war, maybe something else. I'm not sure yet. He was either summoned like a regular demon, in which case he would have been something like a curse (or whatever it was) from a game that featured a bunch of brutal death scenes for the main character, and according to the plot, she went to her father's house, but some terrible shit happened there, and in the end, she survived but found herself pregnant. (SORRY, I DON'T REMEMBER THE TITLE) OR he was created. They mixed a bunch of religious and pagan paraphernalia, killed a bunch of people in brutal ways, dumped them in a pile, and uh... That's how he came to be. In addition to this option, I like to think that he didn't have any expressive character traits or a clear appearance before, and he adopted his fierce grotesqueness and artistry, as well as his appearance, from Mona when he possessed her, exaggerating her character. I think when possessing other people, he would also adopt something from them, eventually turning into a multifaceted and uncontrollable thing.
