This time, I’m not here to compare whose Satan is the better design, because it’s obvious that the two artists drew Satan from different periods.
Doi’s version of Satan is based on this image, depicting Satan as the Great Red Dragon from the Book of Revelation.
In fact, Kaneko also drew this version in Persona1:
On the other hand, Kaneko's SMT Satan is "the Serpent of Eden". Although it looks like a massive, alien-like creature, you can tell from the side profile that it’s actually a giant serpent with wings. Since the serpent was only condemned to "crawl on your belly and eat dust" after being punished by God, it is a common interpretation that, prior to this fall, the serpent possessed wings or limbs.
As for its breasts, I think it’s highly likely that Kaneko referenced these specific paintings. In certain artworks, the Serpent of Eden is depicted as a woman with a snake's body. So, when you look at it that way, a depiction of Satan with breasts isn't strange at all.

In conclusion, this is my point. There is nothing strange about Kaneko's Satan having breasts. SMT Satan is a fusion of two distinct depictions of Satan: the winged serpent and the woman with a serpent's body. He then reimagined this concept using H.R. Giger's "Alien" art style.
