Doug Dimmadome was aware that Chester and AJ were Timmy's friends as he saw them all together with Elmer and Sanjay in the "Odd Odd West" episode so I am theorizing that Doug told them to give Timmy the milk directly so he could drink it because he was aware of their friendship.
