Was just on my 2nd playthrough for Fallout 4 and was dying from this lol. Basically what was going on was that I have an unlimited companion mod active (I bring Curie EVERYWHERE cuz I love her). So I guess the game registered Curie as Ada because of Curie being my primary companion and whatnot- (But programming shit aside this bug is amazing lmao) Here some examples-
