Jaw Reconstruction following Xanthoma Removal This young boy developed a swelling on the right side of his lower jaw around a year ago. There was gross asymmetry of his face. His family had taken him to multiple hospitals where biopsies had been obtained, but no satisfactory diagnosis had been made for the patient. He continued to suffer with increasing swelling and discomfort. The patient had finally been referred to our hospital for management. A biopsy was obtained, which returned with the diagnosis of xanthoma. Xanthoma is a rare soft tissue and bone tumor, which is benign in nature but locally aggressive. This will lead to extensive damage where there is involvement of the bone. All affected regions of the ramus of the mandible were trimmed with complete removal of the xanthoma. The involved molars were also extracted. This resulted in an extremely thin jaw bone following complete removal of the lesion. Rib grafts were obtained through a submammary incision and these were utilized to reconstruct the jaw with screws. He was instructed to return in three months following surgery. Imaging studies were obtained upon his return and there was no recurrence of the lesion. There was adequate consolidation of the grafts and formation of new bone at the reconstructed site. His facial symmetry was also reestablished.
