I don’t require that crossovers make sense, because sometimes that’s not the point, but for myself I like to square Pitt/ER crossovers that bring John Carter’s babyfaced self into Pittsburgh thus:
John Truman Carter III was adopted.
Michael Robinavitch’s mom bounced when he was a kid. She got pregnant again. This time, she abandoned her baby.
Maybe the Carters were expecting a second child and it didn’t live. Maybe the reason was a bit scandalous (what if Carter’s mom’s drinking problem didn’t start with Bobby’s death)? So they quietly adopted a baby and little Johnny T. Carter III never knew.
If we carry ER events forwards into the 2020s, maybe Carter’s parents tell him after his gamma dies. That explains one childhood incident where Bobby was insisting that John was adopted, just to make John cry; it’s one of the few times their mother intervened.
Maybe Bubbe Robinavitch got a postcard telling her that she had another grandson out there, but she didn’t have the funds or resources to follow up.
But one day 23&Me or some other comparable site comes online. People talk about finding far-flung and long-lost relatives through it.
Michael Robinavitch takes the test.
John Carter takes the test.
They find each other.
Now Carter knows he’s Jewish by birth (mazal tov). Now Robby knows he’s not alone in the world. They both get to figure out what family means when you’ve found each other as adults. Their lives have been so different but they both have more in common than they ever expected (ER doctors, bisexual, disastrous dating histories).
And Jack Abbot has to figure out how to manually forget that he knows what Robby looked like as a babyfaced twunklet because he really wants to keep it classy and to not fantasise about fucking the Brothers Robinavitch.
(Meanwhile I like to think that Jack Abbot is a rogue Cody whose identical twin, Andrew, shows up and gets heart eyes for Robby’s baby brother. Because I have the heart of a daytime TV soap writer.)
Because let’s be so for real, John Carter III does not look like a guy whose family tree has always been allowed into country clubs. You can put that boy on a dressage pony but come on. Come on. In what universe does that beautiful nose come with a pure WASP pedigree. He was raised amidst blue blood privilege but something deep in his bones whispered “boychik, you should be a doctor.”

except as he's picking it up he overhears the conversation and he stops, he always has time for some gossip. he's trying to piece together what sparked the story as he listens in and chuckles besides carol and susan. something about a guy outside?
and then 'scary' and 'leather jacket' click together and he drops his chart and runs outside because 'omg omg omg my boyfriend is here'.
it takes everyone a second to figure out what's happening— mark almost running after him just in case—, but once carter jumps into the man's arms and leans in to give him a chaste kiss, they realize who it is.
the mysterious "girlfriend" (and why had they all assumed that?). the one that'd had carter giddy and over the moon for a good few weeks now. the one he'd been tight-lipped about no matter how much they teased him, even if it brought a smile to his face every time the subject was breached.
they watch the man protectively wrap his arms around carter. the way his features soften and carter's face lights up with that lovesick expression they've all become accustomed to.
mark and doug can't believe it. susan hits them over the head when they make a dumb comment. carol is trying to supress the urge to go out there and give whoever that is a talking to about taking care of their boy. chuny is trying to catch malik's attention on the other side of the floor. jerry can't wait until someone tells benton about this.
kerry wants everyone to go back to work.

Popecarter miscommunication angst but the miscommunication is during sex
Carter likes to be ordered around, rough housed, choked a little. And Pope… is hesitant.
He’s not the best at being authoritative, especially not with someone as sweet with him as Carter is. Same with being rough. To him, Carter is one of the few precious things in his life, and it doesn’t feel good to treat him like that. The choking is okay as long as Pope is looking somewhere else, or has his eyes closed entirely.
But he doesn’t want to say that and risk disappointing Carter. He’s so perfect. Perfect to the point that Pope is certain their time together is limited, and he doesn’t want to cut it even shorter by not pleasuring him correctly.
So instead of saying anything he lets the guilt and shame of tainting someone so delicate brew, even if Carter is literally asking for it.
After a few months Carter asks if there’s anything Pope wants to try since he never offers that sort of information. It takes a bit of coaxing, but in the end Carter convinces him to take the lead, do as he pleases, and Carter will speak up if there’s something he doesn’t like.
Based on Pope’s unwillingness to share, Carter’s expecting to get tied up and draped from the rafters. Instead, it’s all gentle touches, and missionary under the covers.
That’s when it clicks, and now it’s Carter’s turn to feel guilty.
