I assume you're talking about the version where Floyd returns and is mutated accordingly, right? Or are you also including the bonus story where Floyd is mutated after Brozone and the Turtles reunite?
Because either way, Berry would be seriously worried. The whole thing with the Oozquitos running around town and mutating innocent (and not-so-innocent) people was a problem in itself, and raising a mutant Ramon while dealing with her own mutation was quite a challenge. But what if someone else she used to know had been mutated and, to top it all off, wasn't quite in the right place?
Yes, Berry would be very worried either way.
In the bonus story after Brozone reunites, she'd probably also try to take care of Floyd with everyone else and try to get his brain to react to familiarity while Draxum works on a retro-mutagen. And in the version where Floyd returns earlier (but not the one where he reunites with Branch to raise him, but the other one where he spends a few years as a vagrant and desperately searches for Branch), she would definitely use all her efforts to catch the wild Floyd and find out what the heck happened to him so she could try to help him. Like, yes, seeing Floyd after so many years would be a huge shock, but seeing him as an out-of-control bat? Obviously, she couldn't leave him like that at the risk of him hurting someone or someone hurting him. So as soon as she finds out about the situation and sees him, she gets to work capturing him with Branch and the Rise Brothers and looking for a way to turn the boy back to normal. Or at least, make him remember them and act like a normal person again, but…
As I said, it would be very difficult and time-consuming for Floyd to regain consciousness on his own, so he would spend most of the story acting like a strange pet, but Berry would take very good care of him until they finally figured out how to resolve their situation.
And yes, Floyd would definitely recognize Berry if he saw her. I'm still not entirely sure what Berry's mutation would be in this universe, whether it would be very mild or very drastic, but in any case, Floyd would be able to recognize her instinctively by the small details, just as he would with Ramon. After all, he's known Berry for much longer than Branch, so recognizing his childhood friend wouldn't be that difficult. And Floyd would be just as attached to her as he would be to Branch, since she's a safe and familiar face.
