Writing up care plans for fake patients last year definitely prepared me for writing real ones this semester! . . Care plans are definitely not a strength for me, not written ones anyways! I constantly make mini care plans in my head and did it as a PSW/CNA/PCT all the time, if you see someone with reddened/pink skin over a boney prominence I immoderately start caring for it and thinking “I need to reposition this person every two hours” “I need to offload pressure on this area, let’s use a pillow to elevate it” “this breakdown is near my incontinence/damp(sweating), we need to keep it dry with barriers or gauze”. . . I am constantly using my nursing skills and applicable knowledge without really realizing it. I try to put myself in a real patient situation when I have to write these care plans for school, and then go back and find the correct references and citations needed to ensure my work is accurate and not plagiarized . .
