When it came to the Fangpyre brothers, I played with the idea that their double heads were a result of having Fangpyre venom backfire and causing them to mutate. So I tried to convey this look that the venom caused a new Fangpyre to sprout from their bodies, sort of; Displacing old flesh with the new as the venom attempted to create a Serpentine where a Serpentine already existed!

Fangdam has his the most grisly where his second “head” seems to be a full set of shoulders and even the beginning of some arms sticking out of his left shoulder. With Fangtom I had this body horror concept where the secondary body growing out of him could actually extend from the ravaged stump to attack foes at another angle, before sliding back into place.
