Not to be that guy but you're thinking of medieval times not stone age.

This is also the medicaly correct procedure for some headaches. Fluid can build up inside the skull and then you need to drill a hole to drain the fluid. And the brain itself can swell and to stop the brain from crushing itself inside the skull you need to remove a large section of skull to let the brain swell out of the head a bit.

I do however agree that performing ether of these procedures outside of a steril operating room is INSANE.
