Headcanon - Akutagawa
Due to his living in the polluted slums as a child Akutagawa’s breathing suffered greatly and as a result he developed chronic and irreversible lung issues which make him highly susceptible to respiratory infections.
His specific condition is known as ACOS: Asthma-COPD Overlap Syndrome. While it isn’t a separate disease itself he has symptoms of both asthma and COPD including:
Difficulty breathing
Frequent coughing
Frequent shortness of breath
Chest tightness
Fatigue
These symptoms tend to be experienced more often and with more severity than just having one of those diseases alone.
A more imminent threat to his health is his tuberculosis (TB). Having contracted it at some point in his youth it did not affect him right away (latent TB infection). However, in his time with the Port Mafia the stress and strain put on his body weakened his already fragile immune system and the TB bacteria became active.
The symptoms of TB which Akutagawa suffers from are:
Coughing up blood or mucus
Chest pain, or pain with breathing or coughing
Unintentional weight loss
Chills
Though he was being treated by Mori the antibiotics he was taking failed to kill all of the targeted bacteria and so it developed into multi-drug resistant TB (MDR TB).

Caleb x MC x Rafayel
Caleb coming home and going to take a shower and when he walks into the bathroom Mer!Raf is just chillin' in the tub.
But Caleb only stares for a moment before getting in the shower anyway. Soon after MC comes to join him, being the exhibitionists they are. When they're done MC quickly steps out and slips into the tub with Rafayel while the water is still warm.
Rafayel pouts as he watches Caleb wrap a towel around himself and start to walk out. "Aren't you two like a two-for-one deal? I'm feeling a little ripped off right now."
After knocking on the doorframe Caleb goes to quickly nip at Rafayel's lower lip. "Deal's almost expired, buuut you just made it."
Caleb and Rafayel waiting for MC to finish up, both lounging in bed. Caleb's eyes narrow as he stares at Rafayel's mouth. He grabs hold of the other's jaw, thumb lifting his top lip to look closely at his teeth, and his own lips quiver. "Is that...blood?"
Rafayel swipes Caleb's hand away and drifts his tongue over his teeth, the taste of iron lingering. "So I bit down a little hard" he says, now preoccupied with how strong that taste still is. "To tell you the truth, I don't think she minded one bit. Don't think she even noticed."
