I donât know how the healthcare system in the Netherlands works AT ALL, but letâs get one thing straight about the US: healthcare is NOT cheap here.Â
AMERICAN HEALTHCARE SYSTEM 101:
Healthcare in the US is more expensive than anywhere else in the world
We believe the lie that we have the best healthcare in the world. Our rates of obesity, premature birth, maternal/fetal mortality, and life expectancy all point to the fact that we are way behind other developed nations.Â
College and graduate medical education in the US is more expensive than anywhere else in the world.
Salaries are high because costs are high, but cost of education is rapidly rising and wages are not keeping up.
Physicians in the US have one of the best paying jobs in the country.
Only about 20% of the money spent on healthcare in the US goes toward physician salaries
We have way more specialists than we need and we have way less primary care doctors than we need.
We have approximately 2.5 doctors per 1000 people in this country, which is way under what we need. Numbers for nurses are similarly low.
Healthcare in the US is treated as a business rather than a service. Thus patients are seen as customers and healthcare is becoming more about patient satisfaction rather than delivering quality care.Â
Doctorsâ salaries are often based on how many patients they see or how many procedures they do.Â
Though doctors do make a lot of money, remember that they also pay a ton out of that salary in taxes, student loan payments, professional fees, and licensing requirements.Â
Doctors work hours unlike most other careers and are not paid on the basis of their time. Most other jobs have limited hours and lengths of shifts, whereas doctors routinely work 60-80+ hours a week (yes, even past residency).
Reimbursement for services is a very complicated system in the US. Itâs based on how complex the care is thatâs being given, and procedures are valued higher than chronic illness management. Doctors/hospitals can theoretically charge whatever they want for their services, but how much money they actually get is based on what the insurance companies and Medicaid and Medicare are willing to pay.Â
I had a patient try to leave AMA (Against Medical Advice) Friday night because he was so scared that his insurance wouldnât cover his hospitalization for a traumatic pneumothorax when he was assaulted at the grocery store and that he would make his family broke by staying ONE NIGHT.

When I was doing my residency in New York, a patient came in 18 weeks pregnant and very, very sick. The only way to save her was to terminate the pregnancy. We were in a Catholic hospital ⊠I vividly recall my director of obstetrics and the chairman arguing with the nuns. They said, âWell, the babyâs only 18 weeks, itâs going to die.â They felt very strongly that we could not do anything, but they would be okay with us transferring her to get care elsewhere. The director rode with the patient in the ambulance because he was afraid that she would have seizures. She was in her early 20s, and she already had a kid. That really got to me. How could we let this mom die and leave her child behind when we have the means to take care of her? I said to myself: âI never want my hands tied behind my back like that again.â I used to travel; these days Iâm mostly in Georgia and Iâm a backup physician in Alabama. In Alabama, my patients tend to be poor and young. The youngest was barely 12. She went to play with a classmate, and there were older boys over ⊠When her guardians brought her in, I was reluctant to take care of her in an outpatient setting because we couldnât sedate her. I went to the local hospital and said, âSheâs just a baby. Sheâs suffered enough. Please, can we put her to sleep.â Everybody was onboard. Things have changed so much I donât know if I would be able to get away with that now. The most frustrating thing for me, especially in the Southeast, is seeing so many women who are not empowered to take care of themselves. Especially women of color. You hear things like âI was told Iâm too young for an IUDâ when we know thatâs not true. They need to know what their options are. Iâm Haitian-American, and the part of me that is extremely cynical wants to say, Well, itâs because these are black women. But I really think itâs a matter of poverty. It just so happens that the face of poverty may be black. A few weeks ago, a woman came in for a medical abortion. As she was about to take the pill, she asked, âDo you think God hates me?â And I said, âNo, he doesnât hate you.â She said to me: âI tried so hard not to get pregnant. I told my boyfriend to use a condom, but he refused and forced himself on me.â If you overturn Roe v. Wade, whatâs going to happen is weâll go back to the way it was before. Every state for themselves. And best believe that the conservative states are going to try to outdo each other. Poor women will suffer. Poor women will die. Thereâs a generation of abortion providers who are more willing to be vocal about the impact of these different legislative measures. I tell my learners, âI donât expect you to provide abortion care, but I want you to support your co-worker if they say, âHey, we need a piece of legislation.â I want you to stand behind us. But most importantly, I want you to be able to counsel and educate your patient in a way that respects her decision.â If I can train 500 providers who are compassionate and willing to respect and help their patients, Iâve done my job.
