To better explain my vision, here have this:
Opening with Dead! honestly feels wrong. I've always treated it as being part of The End, but what's happened here is that the Patient suffered a bit of a fakeout death, having flatlined but being recusitated. However he knows about a complication with his heart, having only two more weeks to live.
Teenagers is the patient kinda scoffing at his diagnosis. He's done jack-shit with his life and he's accepted it, for now at least. Maybe he's putting up a facade. You could also try to work it in as an explanation of The Patient being groomed to be a violent, angry person towards the very society that made him that way. (Credit to /u/underdogger from reddit about 4 years ago and for some of the other stuff here too as long as the original album is involved.)
Party at the End of the world is keeping up this energy, the Patient meets with his friends one last time, his friends expecting a whole big night of debauchery. Some debauchery has been had, but the patient is abstaining and his friends not being entirely supportive of his goal to go cold turkey seeing that he only has days left to live (so what would that matter?)
The Sharpest Lives, the patients girlfriend breaks up with him basically
--Flow of Consciousness Suite- (Could be happening all at once, or over the course of a few days.)--
My Way Home Through You is the patient basically verbally arguing as the breakup occurs.
I Don't Love You is the patient's response to the breakup.
How I Disappear is the patient realizing once again, he only has days to live, and there are fewer people now who'd care to remember him, perhaps nobody.
Not That Kind of Girl is the patient being bitter about his Ex.
Emily is the patient feeling guilty for his actions with his Ex, realizing he managed to screw shit up so amazingly horribly with just days left in his life.
Kill All Your Friends is the Patient accepting his death, or at first things seem. By the end of the song it seems the patient's mask is slipping and he is in fact, terrified.
ACT TWO -Dawn of the Final Day-
The End is, the end. The patient has found himself in his deathbed. He understands he's going to die and that quite possibly no one cares. The song however ends with him pleading for someone to save him.
the cut to Cancer is terrible and I hate it I hate i hate it. But this is the chronology. He's afraid to have his family see him in this state, afraid that this is all he'll be remembered as.
Mama is the patient realizing that even possibly his mother doesn't care for him. It is revealed though by the end that his mother does care for him and is terrified of seeing him die, despite everything her son has done.
All the Angels is the patient's Ex barely making it in time to meet with him before he dies. Revealing she feels like she carries some of the blame for his death.
In Welcome to the Black Parade, Death has come for the Patient in the form of the patient's happiest memory, being when his father would take him to see parades. Death manifests as The Black Parade, guiding the Patient to the afterlife.
ACT THREE -The Afterlife-
In Heaven Help Us, the patient realizes he is likely being guided by The Black Parade to hell.
House of Wolves continues on the road to hell, mocking people who hide their sins behind their fake faith. Everyone sins and everyone goes to hell so why fake your innocence? (Thanks again /u/underdogger)
Sleep has the patient coming to the conclusion that if he's going to hell, there's no point in feeling remorseful for what he's done and sees himself as a monster and now that he's dead he can't make up for who he was while he was alive.
In Disenchanted the patient reflecting that he did a whole long lot of nothing with his life, that it was just a "lifelong wait for a hospital stay".
In Famous Last Words, the patient is rescucitated to once again find his Ex in the hospital, professing his undying love for her but doesn't know if it's a dream or not. It's vauge whether or not the patient has actually been given a second shot at life or not by death. (/u/underdogger)
