My aunt-in-law does this. She asked for an itemized bill once and saw that they charged her $25 for a single bandaid for her son’s 3 stitches. So she marched over to RiteAid, bought a box of bandaids, marched into the billing office, slapped one band aid on their desk, and demanded they take the $25 off her bill since she reimbursed them one bandaid. There is a world of difference between self advocacy and "being a Karen" Standing up for yourself against the healthcare system, seeking a second opinion from a doctor who isn't treating you when you have a problem, even sending back an order that's wrong at a restaurant ISN'T BEING A KAREN, it's just pointing out a legitimate problem. A Karen is loud because they feel entitled to something that they aren't entitled to and shouldn't be entitled to. You deserve adequate care, and it's not outrageous to say so.

Harry potter fanfiction itself didn't lead to anything. If Harry potter wasn't around, these groups would use some other popular media property. They wrapped these things in Harry Potter aesthetics because for nearly a decade it was one of the most popular pieces of media on earth. I also generally don't like to focus on whatever pop-culture thing is wrapped around these groups. It distracts from more meaningful discussion. It also leads to people comparing Snapewives and Zizians in ways that are not necessarily useful or material.

A lot of people cover groups like these from the angle of internet anthropology, or public interest journalism. This isn't inherently bad, people have done it well, but inarticulate discussion can lend itself to sneering internet freak-show behavior. Or more commonly, relegating the topic to an incurious fun fact.
