Setup: There are gods and powers friendly to mortal kind, reflecting and encouraging beliefs and ideologies coherent with the desires of their worshipers. Then there are the “outer” gods, entities who pursue ends heedless of their effect on the congregations they govern. A war god may encourage slaughter and cruelty in the pursuit of victory, but an Outer god of war may demand their followers poison their wells and salt their own fields so they have no option but to slaughter others for sustenance . Only the mad or short-sighted hitch their fortunes to such reckless entities, as they are actively shunned by most pantheons who jealously protect the fragile words that have come to worship them, and encourage the destruction of outer-god cults for fear of the destruction that follows in the wake of their calamitous rites.
Often depicted as a black goat, a twisted tree, or as a faceless and gravid satyr, Shub-Nuggurath is patron over all things that grow and rot without concern for others. Invasive species, cancers, plagues, and poisonous creatures are her chosen heralds: the “thousand young” so often mentioned in her litanies. Her influence sees strange, alien gardens growing in unexpected places, as her toxic vitality suffuses her worshipers and the land they dwell upon, heralding new and unwelcome forms of growth.
Adventure hooks:
Desperate to defend his homeland and traditions from the slow encroachment of urbanizing empire, a druid breaks from his pacifistic circle to invoke Shub-Nuggurath, sowing her poisons through the forest and raising an army of blighted plants and rabid, mutant animals.
Renowned for her miracle cures, a well connected doctor in service to the royal court secretly invokes the Black Goat in order to perform her medical miracles. When traditional skills and magics fail, she has the outergod grow new “improved organs”, implants squiggling symbiotes, or transfuses sickened flesh with alien humors. When it’s discovered that some of her follow up medicines contain poison ( necessary to curtail the mutagenic spread of her initial treatment) she’s forced to flee the capital, leaving her patients behind to “bloom” in the black goat’s care while she’s hunted as a would be assassin.
Among their many other enticing attractions, a traveling circus boats an extensive menagerie of beasts from exotic lands. sadly the menagerie is closed to visitors at the moment, as one of the recent acquisitions seemed to be carrying some sort of sickness that’s spread to the other animals. It’s not a day or two into enjoying the circus’s other attractions before the party must leap into action and protect the revelers from tigers and elephants and a whole zoo of outer-god warped beasts.

Bloodborne Monster Manual - Forbidden Woods
I wrote a homebrew way to reasonably implement phases to D&D combat. I took a bit of inspiration from Matt Colville’s Villain actions from his “Action Oriented Monsters” video.

Inadvisable Dungeons & Dragons adventure premise #137: a mysterious benefactor hires the party to undertake an extraordinarily dangerous mission; upon their return, the benefactor is revealed to be a ghost who used the party to complete their unfinished business, and passes on to the other side without paying the agreed-upon reward. In the following session, the party journeys to the afterlife to track down the deceased deadbeat and get their fucking money.
I can’t see the point, you can’t take it with you.
It’s D&D – you can absolutely beat a ghost until money falls out.
(More seriously, most of the cultures that D&D’s settings and cosmologies crib from include at least some notion of grave-goods and/or material offerings to the dead, so there’s a non-zero chance that you can, in fact, take it with you.)
New form of joke: telling someone to roll an ability check for something that obviously doesn’t need that type of check

You say joke, but I like to use it as an opportunity to genuinely fuck with players. “I want to see if the dead body is anyone we know.” “Roll acrobatics.” “11?” “The body seizes you by the wrists and lunges at your throat with its teeth, howling like a demon. You are grappled. Roll initiative.”
Or, “I want to check the chest for traps.” “Roll performance.” “Okay, uh. 17…?” “You start whistling quietly while you go about tapping, poking, and examining the sides of the chest. It’s a pretty catchy tune you picked up a couple days ago from the bard.” “Alright, neat, and the chest?” “The chest starts humming along.”

There are actually a couple of tabletop RPGs based on Simon Stålenhag’s work, if you have that particular intersection of interests. Tales from the Loop is pure 1980s cassette futurism with kid protagonists and a largely family-friendly tone, while Things from the Flood speculatively advances the same setting into the 1990s and presents a world in which cassette futurism’s utopian promises failed to manifest and now everything is slowly falling apart; as you may have gathered, the latter’s tone is rather less kid-friendly!
Cobbled Wings- Wondrous item, Uncommon (requires attunement)
A strange vest which encompasses a framework of metal and fabric, which expands at a moment’s notice to become a glider, often with wings of stitched leather or sailing cloth. The wings of this glider stretch to a span of 10 ft. on either side, and can be opened or collapsed by using a bonus action. If you fall more than 10 ft. while wearing the glider, you can instead use your reaction to open the wings and slow your descent. While the wings are stretched outwards, you glide instead of falling, letting you move 5 ft. in any direction for every 5 feet you fall, and take no damage from falling. The wings are also magically enchanted to allow for short bursts of actual flight, and have 2 charges. While attuned to the wings, you can use a bonus action to expend a charge and gain a flying speed of 60 ft. until the start of your next turn. The wings regain 1 charge each day at dawn.
