I have been digging deep, and I think I found the first-ever audio roleplay.

The Whistler, from Colombia Broadcasting. Starting in 1942, the narrator refers to the audience in the second person, speaking to them as though they are witnessing the story in-person as a silent character present alongside the characters.

It doesn't include implied dialogue like modern audio roleplays do, instead relying on dialogue from a narrator to give the Listener presence. Sometimes the Listener's actions are described by the narrator, with the narrator occasionally criticizing the Listener for getting into a dangerous situation. The narrator will also get the audience into character by describing settings, and implying that they've been there before. Sometimes the Listener is meant to roleplay as a voiced character, instructed to imagine their point of view, memories, and inner thoughts.

It's very, very different from the modern format but this might be like my paleolithic ancestor.
