HRT exists in D&D just as it does irl, alchemists have discovered a way to extract or chemically reproduce testosterone and estrogen for various species. Sure, more immediate, magical ways of transitioning gender exist, but they are either super expensive or super convoluted. By comparison, taking a pill/tonic every so often from your local apothecary is much cheaper and straightforward. The only real issue is making sure you got the right kind of hormone for your species.

Her tone changed immediately as her eyes came back into focus, recognizing the ghoul's gravely voice and the weathered face that gazed down at her. "... I have been stupid, haven't I? Since you probably had to save my foolish self. Again."

Grunting in pain, she tried to sit up. Her senses were dulled, but the danger seemed over. "How could I be so stupid?" She tried to answer his rhetorical question. "I don't know at all... I was on a job, and some manner of robot attacked me, so I -" she punctuated her speech with a right jab in the air. "I just traded for a modified power fist, to fit my amputated hand. I suppose that robot gave me an electric shock for my trouble."

She felt a sheet of cardboard under her body, a common kind of improvised bed in this world. "Must have been unconscious a while. Apologies, Mr. Ares, to put you through that. Many apologies. And... Thank you." She paused, wincing due to a throbbing headache. "... Where are we? Still, ah... On the job?"
