The use of Dolls as a motif in this episode is rather interesting
Mamoru Oshii in the script for Ghost in the Shell 2: Innocence described the psychology for the fear of dolls thusly
[Itās] the uncertainty that perhaps something that appears to be alive actually isnāt and the uncertainty that on the other hand something that doesnāt appear to be alive actually is. If you want to know what makes dolls so unnerving itās because they are modeled after humans. In other words they are man created in their own image. [It] creates the fear that the human phenomenon is fundamentally devoid of meaning and purposeā¦that humanity can be reduced to simple mechanical parts.
In the context of the story the use of the motif of dolls reflects the fear of being replaced. To be cast aside in favor of something new. Broken mechanical parts swapped out for newer ones that last longer and work harder.
Asuka as a character values her skills as a pilot because they give her a sense of self worth through others dependence on her. She takes pride in her competitive spirit and in her high synch ratios. She loves the fact that others depend on her to defend them against the Angels. However, she is dependent on others for her sense of self worth despite her personality. After all, human beings are social creatures and we depend on each other for both social, physical, and mental needs in different ways. You simply canāt live a healthy lifeĀ āon your ownā.Ā Without the ability to pilot the EVA, she is cast aside like a broken doll or broken parts in a machine by the power structure of NERV and SEELE which soon finds a new ātoyā to replace her with in their game now that she no longer has any value to them.
As a side note the language that Gendo uses when discussing the pilots is rather interesting, in the subtitles pilots are rarely if ever referred to by their names, the exception being Rei. Even Rei outright states that if she dies she can simply be replaced. When Shinji shows up heās referred to as āthe spareā. The pilots are simply interchangeable parts in the command structure, kicked out when no longer useful and replaced.

I just realized how Rose died.
Iāve only been in the fandom for a few months now but I just realized how Rose Quartz died. See the show and the fandom always talk about how she gave up her physical form to be half of Steven. And I think a lot of people get confused by what she meant.
Like Iāve seen a lot of fanart, metas, theories, etc. which suggest that Rose actually physically gave up her physical form so that she could transform into part of Steven and keep him alive. In other words, Steven is her reincarnation. (Well, heās half her reincarnation and half of Gregās DNA.)Ā
Which never made sense to me but then again Iām new to the show, so Iāve just sort of gone along with it.Ā
But now Iām here to present you all with a simpler (and ironically, darker) theory.
Steven isnāt a reincarnation of Rose. Steven is just Roseās child. Which is still impressive and groundbreaking since heās the first human-gem hybrid. But heās not a human-gem reincarnation. Heās just their child. All of his powers are inherited from his gem DNA, not from being a reincarnation. Iām going to go so far as to say that even his gem was a product of DNA and not a symbol of the fact that heās Rose 2.0.

To give birth to Steven.
Yeah.
According to Rebecca Sugar,Ā āRose shapeshifted a womb for him to be born from.ā
Pretty impressive Gem magic.
But hereās the thing.
According to Amethyst in Stevenās Birthday,Ā āSteven, you canāt just keep stretching forever. If you hold it too long, you could really hurt yourself.ā
Hmmmā¦fascinating.
It appears that if a gem shapeshifts for a prolonged period of time, theyāll have severe consequences. And we see this even further when Steven doesā¦
ā¦that.
Now for some odd reason, that turns Steven back into a baby, but thatās not what Iām getting at. Iām getting at the fact that Stevenās body took an extreme physical toll after just a few hours of shapeshifting. Because, as Amethyst said, prolonged exposure to shapeshifting can cause someone to āreally hurt themselvesā.Ā
Rose Quartz shapeshifted for nine whole months. And thatās not even counting the stretching she did to push Steven out.

When Rebecca Sugar and the show talk about how Rose Quartz gave up her physical form to give birth to Steven, they literally mean that Rose Quartz stretched herself out for such a prolonged period of time that her body (ie: her physical form) eventually faced the consequences and withered away into nothingness.
Stevenās Birthday wasnāt just a filler episode. It was the key to showing us the process by which his mother died.
