My buddy read an article about octopus intelligence. It was feeding time, and the handler dumped some shrimp into an octopusâ tank. Then he went into another room and sat at his desk.
A while later, a shrimp was tossed onto his desk.
The octopus, upon finding one bad shrimp in the lot, had grabbed it, escaped its tank, crossed the hall, and threw the expired shrimp at its caretaker. Not only does this showcase their problem-solving capabilities, but also that it could have escaped at any time. It just broke out this time to chuck an off shrimp in indignation at its handler. Thatâs not just intelligence, thatâs a human-like reaction. Kinda make you wonder exactly how smart these guys can beâŠ
OH MY GOD
I went to the aquarium once and we had a tour and we walked past the octopus tank and it was duct taped shut so I asked why and the guy was like. âWell, we had a problem before because these fish were disappearing randomly at night and we had no idea why. Turns out the octopus had memorised the night guards rounds and would creep out of its tank, crawl across the floor to the fish tank, have a little snack and be back in its own tank with the lid shut before the guard came back.â they are super smart

Once I went to the aquarium where they had a baby pacific red octopus in a tank. I had gone there to work on a few real life sketches, obviously I wanted to do one of an octopus. So I kinda just kneeled in front of the tank, and started sketching. The octopus didnât mind, he sat happily. Then, 5 minutes later, he started moving to the front of the tank, where I was. This tiny octopus faces me directly and starts posing. I donât know how other to explain it but he started curlung his tentacles in this really graceful way then wouldnât move for a few minutes. Then again, a new pose. That tiny cute motherfucker knew I was drawing him.

Do not bitch about the ER wait if you are not - in fact - dying
If you have to wait hours upon hours waiting for a room in the ED, please consider the people fighting for their lives who are utilizing resources that are causing the wait. Please consider the people who lost their fight for survival - probably not even 500 feet from you - who would have liked to have the time to wait. Please consider that if it was your loved one fighting to live, how much you would want staff to pour everything they had into saving that life - even if it meant that others might have to wait. Yes, you may actually be having an URGENT issue that may warrant admission. Yes, you may need medical care within the next day or suffer the health consequences. But if you CAN wait for a bed, you often will be waiting. And, even if you get a bed in the treatment area, you are likely going to have to wait there too because doctors might be busy with lots of traumas. In short, if you are able to breathe, are not hemorrhaging uncontrollably, are oriented (know who you are, where you are, and what year it is), and are maintaining your own pulse, you do not need immediate bed in treatment area. If that changes, notify the nurseâŠ.preferably before you die. It is very rare to die in the waiting room (it CAN happen) due to the triage process that is always being improved. But the general rule is that if you CAN complain about the wait, generally you are doing much better than the people who are taking all our resources.
Iâve said it once Iâll say it again: if you are waiting be happy it means you ARE NOT actively dying.

Tonight I had a patient come in complaint of Abd pain. No big deal really routine.
More so we thought.
Doctor comes in goes through his series of questions and starts to palpate her abdomen. She has some pain/ sensitivity in her lower left quadrant. So doc decides to do a pelvic exam.
As he is explaining to this patient whatâs going to happen next. She very casually states â okay but be careful I have leaves growing out of my cooterâ.
You what?
Doc at first was skeptical has her change and we come back for the exam. And holy fuck was she right. LeavesâŠ. growingâŠ.. thatâs not the weirdest part.
Upon further inspection we realize she had stuck a potato up there. Easy to remove so as Doc is going to remove it she goes â you have to leave it there! My insides are falling out!â
She had a prolapsed uterus and had used the potato to hold it upâŠ.
Nothing surprises me anymore.

Yeah, thatâs messed up.
Wow, itâs really rare I see something cross my dash thatâs actually directly tied to my life on a personal level, but yea, Iâm Diabetic Type 1 and this is a problem.
Iâve been off insurance for the last three or so years and have been working around through channel I can to continue to obtain insulin and supplies for my insulin pump (of which is currently a problem, fun) for free or at reduced costs.
The Lilly Cares program is one I heavily endorse if your insulin is a Lilly product. Theyâve been incredibly helpful to me.
Please spread the word on this. There are a lot of young Diabetics like myself that do not have a support system, do not have insurance, and do not have jobs. Insulin is literally a life-sustaining medication for T1 Diabetics. Please do not just ignore this.
T1 diagnosed over 20 years ago here. Back when I was out of work in 2009 I contacted Lilly Cares and I swear I would not be alive if not for that program. T1s need insulin to live. Daily. Our bodies do not produce the hormone because our immune systems backfired and killed our pancreasâ islet cells.
These days one bottle of insulin costs me approx $600 (before insurance) and lasts 2-3 weeks, tops. Less than ten years ago the cost was closer to $200/bottle. The insulin manufacturers keep âtweakingâ insulins like Humalog so the patents can be extended(*), so we donât even have access to a generic option.
The price gouging on insulin in this country is cruel and damn disgusting.
Please, please boost this info. It WILL save lives.

It's Time To Simplify Cervical Spine Clearance!

Cervical spine clearance is another one of those tasks that everyone seems to do their own way. Most trauma centers have an algorithm for clearance, or even two, like my center. But anytime different clinicians or centers do the same thing in different ways, it means we donât really know what weâre doing.Â
It basically means that the hard data is not there to dictate what we truly should do. So there are two alternatives:
Wait for good data to become available. Unfortunately, this can take forever.
Extrapolate from any existing data, and fill in the gaps with our clinical experience to come up with something that works and causes no harm.
The protocols in use at Regions Hospital are based on #2, and have been in place for over a decade. But now, we have a good example of #1 to work with.
Fortunately for us, cervical spine clearance has been evolving for decades. And as technology has improved, so has our ability to miss fewer and fewer âsignificantâ injuries. A multi-center trial published this month provides one of the final puzzle pieces to help us settle upon a uniform cervical spine clearance guideline. It was a prospective look at intoxicated patients after blunt trauma, who canât always participate in the process of clinical cervical spine clearance.
This three year study took place at 17 centers and specifically looked at the combination of clinical and radiographic clearance in alcohol and drug intoxicated patients. Over 10,000 patients participated in the study. There are some limitations, of course, when so many centers participate. But the pros massively outweigh the cons.
Here are the factoids:
The overall incidence of cervical spine injury was 10.6% (!)
30% of patients were intoxicated (19% etoh, 6% drugs, 5% both (also !)
Intoxicated patients had a significantly lower incidence of cervical injury (8% vs 12%). (Donât get any ideas about the old adage about being relaxed when they crash. This probably represents lower speeds involved.)
For intoxicated patients, sensitivity of CT scan was 94%, specificity was 99.5%, and the negative predictive value (NPV) was 99.5%
The NPV for clinically significant injuries in intoxicated patients was 99.9%, and no unstable injuries were missed by CT (100% NPV) (!!)
When CT was negative, being intoxicated led to longer time in a collar (8 hrs vs 2 hrs)
Bottom line: Fear of clearing the cervical spine without a clinical exam, or in obtunded or intoxicated patients, is primarily due to old anecdotal reports. And much of it is not first-hand experience, but rumors of othersâ. What is finally becoming clear is that it is okay to clear based upon radiographic findings alone.Â
Tomorrow, Iâll provide my version of a new, unified clearance protocol based on this work.
