Dean was supposed to be tattooed but there was no budget, we all know this.

But HEAR ME OUT.

They could have heavily tattooed him up until season 4, where Cas, having to basically reconstruct him on a molecular level, would have brought him out of hell brand new, not a tattoo in sight, avoiding the problem of tattoo continuity.

Bonus points thinking about how Cas studied Dean every nook and cranny to be able to bring him back exactly as he was (think of that what you please)

Bonus bonus points (as pointed out by chronic overthinker @promisedcos) Cas knowing Dean so intimately he always saw beneath the tough exterior, bringing him back without the tattooed armour he had built for himself.
