i've actually been working on just such a list because a previous anon a few weeks ago asked me this and then i was languishing in the task because I felt the need to go back through the entire tag... i just realized I can reblog this with additions later. ANYWAY.
@grandfather-clock just did a big rec list here so go read that. there are some repeats here but everything on that list is good. i have focused this rec list on things that have some combination of freak behaviour (emotional) and freak behaviour (sexual). though not everything on this has on page sex. just trust me.

giving in when you change the subject by flightspath. they're SO emotionally constipated in this, just like total inability to meaningfully communicate while simultaneously being totally dumbass devoted to one another. great stuff, very ow.
filling in the blanks and gaps by flitwickslittlebrotha. i LOVE breakup makeup fics, and this one is. whew. they're so messed up and grieving and they love each other so much.
roses between my thighs by orphan_account. this, to me, is a pynch fic of all time. the grief and tenderness here combined with awkward and overly hostile attempts at communication, plus the way this fic hits at adam's control issues and his sexuality is perfect.
made by lovehaunts. the way this fic hits shame, control, and vulnerability is really perfect. this, to me, is a thesis statement about writing kink in terms of pushing characters to their emotional limits.
no grave can hold my body down by anon. this might be the softest fic on this list. another one about shame, vulnerability, and control, but I think the thing it does exceptionally well is exploring Adam's struggle around expressing affection and being loved. It also hits really well on a kind of "new to sex and figuring things out" vibe which is really charming while avoiding going full awkward realism.
show me where to tie the other end of this chain by oorione. the way they want to consume and be consumed by each other is EVERYTHING. also fixing some greywaren problems.
concorde by grandfather_clock. ongoing. pynch sicko magnum opus, an au in which ronan and adam meet as children and everything is better and worse. i really love how extremely sharp they are with each other in this, and how it's so obviously borne out of love that is just barely off the margins of the page.
together, transformed by otherworldliness. aka the surgery fic. which you clearly already know, anon, but I could not let this list pass without including it.
the mess inside by fitztomania. another breakup makeup fic that exhibits some top tier insane behaviour on all sides. i wish it was twice as long as it is.
