FTM Transgender Chest Surgery/Reconstruction.

As many would hopefully know, I am a transgender man. 

I’m in the UK & my top surgery went ahead in February 2015 in Brighton, England.

Before & since then I’ve seen vast differences in results from other Transmen.

I was fortunate, as my surgeon (Andrew Yelland) is considered to be a top surgeon in this field. (See his website at https://www.transurgery.co.uk for more).

My wife & I spent a long time researching the various options & surgeons available to me. 

Andrew Yelland is set apart from other surgeons (at least here in the UK, & from what we researched elsewhere) in that he doesn’t use drains. This lessens one possible area of complication.

Because his specialty away from top surgery is to do with a wide range of chest surgery (including reconstruction), he has ultimately developed as non-invasive a method as possible.

This is one reason he is right up at the top of his field.

I’m including photos to show how my chest looks now. 

When looking for a surgeon, you want to avoid those who merely do a mastectomy, as it is not that at all. Some breast tissue (to allow some sculpting to shape) needs to remain. All too often, too much - or all - breast tissue is removed, giving a far flatter appearance than is wanted & can be pretty disastrous in the end result. 

Nipples need to be grafted back, & often reduced in size. 

These are all things to consider, & any good surgeon will raise them (& other needs plus options available not just possible complications). 

If you are unsure, look around for testimonials from other patients (good & bad). Try to find online support groups (I found help via some on Facebook). 

Look for other transgender men online who are openly transgender, & see if they either document the experience or if they will chat to you.

One of my wife’s cousins children recently went through top surgery, & they spoke to us quite a lot just ahead of it, knowing my experiences. 

I don’t claim to be an expert, but I urge anyone no matter how bad your dysphoria feels, you need to be sure the surgeon is the right one. It’s easy to settle for a result just to get the surgery done, but in my experience talking to others, once the results settle down it can often then be realised that actually, the surgeon didn’t do as good a job as others might have done. 

So, please talk to others where possible. Look up surgeons. Read testimonials. Meet the surgeon. If you have an inkling of doubt, then look again. 

I’m so pleased with my surgery. There is no doubt, by making the choice I did, it was absolutely the right one for me. I also had no complications.

Recoverrryyyyyyy yaaaaayyyy here’s my timeline so far:

25th March 2019 - surgery (A.M.), transferred to ward, in overnight for observations. Drains left in post-surgery.

26th March 2019 - discharged from ward with Sick Note, prescription painkillers, aftercare instructions, and a fetching purple bag to put the gooey drains into so I don’t scare people and/or button the tubes into my pants by mistake (fucking genius).

27th March 2019 - outpatient appointment (A.M.) with same consultant to check that the drains have done enough de-glooping and can be schloobled out. Drains removed, given appointment for dressing removal.

Interim: some faff due to my original follow-up date clashing with everyone being away at a conference resulting in a 3 week follow-up instead of 2-week.

Interim: various calls with the dedicated MacMillan nurses to ask about dressing maintenance, if pain levels/etc are normal, and ask any other questions. Reinforce dressings with Micropore tape on their advice, where edges of the dressings had peeled.

10th April 2019 - outpatient appointment (A.M.), dressing pads removed, gain unexpected minor chest waxing in the process since medical glue does NOT fuck around. Quick assessment of sutures/scars, all progressing very well. Sent off with a further Sick Note, spare plasters, aftercare instructions, and a follow-up appointment note for 6 months’ time. Instructed that if I have any questions I can contact them via phone/email and not to go via my GP for recovery care.

I really can’t fault the service or the people that have looked after me. With the caveat of ‘once I got a first referral’ the process has been pretty damn smooth, and from first encounter (June 2018) to now means that I was waiting slightly less than a year to be seen for surgery.
