For reasons that aren't even clear to me, the sensation of having a full bladder feels really good to me, and especially makes sex/masturbation feel much better and more sensitive.
That's only moderately extreme in its own right, but here's the part that makes this a confession:

When I was a young and stupid teenager who hadn't completely figured out how to get off, but was starting to get horny, I somehow figured out a trick. I would go into the shower and press myself against the showerhead to fill my bladder with water, so I didn't have to chug drinks and wait hours while squirming to fill myself up. Eventually, I realized what a terrible fucking idea this is because of the risk of horrible UTIs and stopped doing that, but I honestly still miss it, it felt so damn good.

If I'm remembering right, there was a chapter where the main character filled up the bladder of one of her students with her own pee by running a catheter between the two of them, then having the girl sit on her stomach to apply pressure to the bladder.
