Dane Whitman became the Black Knight in Avengers 48, cover date January 1968. The issue also introduced his horse Aragon and J.B. Desalvio. They were created by Roy Thomas and George Tuska. The new Black Knight attempted to join the Avengers. He was mistaken for a bad guy, though, and was attacked instead. ("The Black Knight Lives Again," Avengers 48, Marvel Comic Event)
