Lady Stoneheart
Lady Stoneheart--the back-from-the-dead Lady Catelyn Stark--is probably the best known character in the books who did not make it onto the show.
Lady Stoneheart is referred to a few times in AFFC but she actually appears only at the end, in the final Brienne chapter, Brienne VIII. Initially she is described as “a woman all in grey, cloaked and hooded.” But when the time comes to pronounce judgment, Brienne sees her face.
Lady Stoneheart lowered her hood and unwound the grey wool scarf from her face. Her hair was dry and brittle, white as bone. Her brow was mottled green and grey, spotted with the brown blooms of decay. The flesh of her face clung in ragged strips from her eyes down to her jaw. Some of the rips were crusted with dried blood, but others gaped open to reveal the skull beneath.
Her speech is unintelligible to Brienne.
...she reached up under her jaw and grasped her neck, as if she meant to throttle herself. Instead she spoke . . . Her voice was halting, broken, tortured. The sound seemed to come from her throat, part croak, part wheeze, part death rattle. The language of the damned, thought Brienne. “I don’t understand. What did she say?”
Lady Stoneheart dismisses Brienne’s insistence that she had always kept her vow to Catelyn Stark, and that Jaime has changed. Stoneheart is merciless; she wants revenge. Brienne must “slay the Kingslayer, or be hanged for a betrayer.” And Pod with her.
It seems pretty clear to me that Lady Stoneheart--”the thing that had been Catelyn Stark”--is another example of a negative, reverse transformation, just like Gregor Clegane. I wrote about him here:

And the text tells us, in the words of Brienne’s thoughts, that Lady Stoneheart is damned, just like the Mountain. Lady Stoneheart’s sins are nothing compared to the murders of Clegane. But the implacable desire for revenge is the fuel that feeds the fires of conflict and civil wars throughout Westeros. The endless cycle of revenge must end. An alchemy story should end in a Golden Dawn of peace and reconciliation. There is no place for a vengeful Lady Stoneheart there.
In the books Catelyn is consistently hostile, even cruel, to Jon. This scene, from Episode 3.02, where she rethinks her attitude and actions, was entirely made up for the show.
I assume this was D&D’s effort to make Catelyn more sympathetic, to partially redeem her. Given that, it’s not surprising that they would eliminate her resurrection as the unforgiving, merciless Lady Stoneheart.

Sketch commission for Jonjeff2014, Karth with a booze-soaked beanie slapped onto their head- a drunken daze overtaking them as dyed blue hairs would start peeking out from their new hat- changing over wardrobe as Grey-like thoughts would flood their mind. Grey, for a mere human, is quite indomitable in will, particularly when she's invading other people's forms.
Complete Splatoon Obsession
Commission by YeppersPeppers Zeydaan/Isabella riding the new Splatoon craze, and finding themselves a bigger fan than they thought they would be. Neglecting their duties to playing the game- while buying all sorts of merchandise. Friends would notice their recently and colourfully dyed hair would be getting thicker and shinier- and how their near constant quoting of squidese would go from obsessive to being their default way of speaking. And as they started to fully embrace inkling culture by doing full on turf wars- their non-inkling friends would have to deal with the fact they had turned into a full blown inkling. Woomy
