Well, Hiyori is often conflicted about becoming a parent herself. she does want children sometimes, but not out of any desire to become a mother. Her desire is mainly because she knows how good Shinji would be with a child. She sometimes daydreams how nice it would if Shinji was smiling in wonder at his own child, lazily curled up at the engawa of their home with a small baby in his arms… she knows he would be wonderful. His patience and kindness and care towards anyone who doesn’t know their way around the world would be a beautiful thing to witness. She does not think she would make a very good mother.
The hollow is not the only monster inside of her; Hiyori knows that raising a child when she is healing from bitter feelings like rage, resentment and betrayal… and then to give a job to a small child to somehow understand the complicated feelings she has that stem from her depression is the cruelest thing she can do. At least Aizen ensured his experimental subjects stood a fighting chance.
And the other factor is that the traumatic wound she suffered in the winter war, where she was bifurcated, means that her body is no longer the same. That’s perhaps the part that really hurts the most. She never really had a choice whether she can become a mother or not. Someone already took that choice away.
It is fortunate then, that she already has all the family she needs to feel loved and happy. She has her husband and the vizards who have been through everything with her.

Before they even recognised their feelings for one another, the way Shinji used to comfort Hiyori was through ensuring that she got adequate rest. Trying to consistently keep your inner hollow in check, as well as to make sure you didn’t murder your friends - -meant that there were many nights where she would rather stay awake. Losing consciousness meant losing control. But the only way to feel safe enough to lose her consciousness, Hiyori would fight with him until she was settled enough for a nap. She could listen to his heartbeats and she wonders if he knew it that she matched her own erratic beats to his calm, steady ones. It helped her relax and feel safe to have his reiatsu around her. He would never allow her to hurt anyone.
Shinji would often give up his own rest to ensure that she got hers. He still does this to this day- to make sure that she always feels safe in his arms. His reiatsu calms her even in Soul Society, where she was often at edge in the beginning of their courtship.
