Remember that Elizabeth is only 14, even in the Victorian era she'd be considered a child, so mentally it can seem confusing because it is confusing at that age.

She does love Our Ciel on a romantic level, Yana has definitely said that many times in the story and in character design discussions, but she's also deeply hurt and confused.

Imagine her perspective? The boy you've loved since you were little comes back after being presumed dead so you learn to love this new broken side of him for THREE years, only to then find out he wasn't that boy all along but you're in love with him instead of the one you're told to marry AND left completely disoriented over which twin you love? If that were me, I'd be a wreck!
