I've seen this bonus illustration for a while, and reading that Yoshiki doesn't develop any romantic feelings toward Fem Hikaru is, frankly, not too surprising because the manga and even the anime did a very good job letting me (a very dense oatmeal) know that Yoshiki likes men.
(Sidenote: Yoshiki, I know Takeshita isn't your type, but you should hype up your homegirl.)
When I said dense, I'm really the type to be told directly about everything, and this series both subtly and heavily kept bonking me on the head about Yoshiki's interest in men. So I gathered a few scenes and illustrations that got their point across.
Maki asking Yoshiki, "Why do boys stink?" like Yoshiki over here cares that Hikaru/"Hikaru," who commuted by bike and train to school and is all sweaty and dusty from the summer heat, is rolling on his bed and hugging his pillow. (Low-key surprised Yoshiki doesn't have acne with all that outside debris).
This bonus illustration shows Yoshiki side-eyeing Maki who is checking out a raunchy magazine. I see two reactions from this: first, Yoshiki is not interested in raunchy magazine covers, and second, he is absolutely embarrassed and definitely judging Maki and Hikaru's behavior over a magazine. Cause we get it, you like girls Maki, but you need to chill out.
Yep, I'd cringe too, Yoshiki. I'd cringe too.
This anime's original scene shows Yoshiki and "Hikaru" shopping for Kaoru's pool ring. Yoshiki chooses a giant salamander because it's the cutest in his eyes. "Hikaru," an eldritch entity that has only been residing in that body for six months, has a better grasp of what girls would find cute. (Also, it's not even a pool ring, Yoshiki, come on!)
Overall, it's a pretty neat bonus illustration and piqued my curiosity about what type of friendship dynamic OG Hikaru and Fem Yoshiki would have. Maybe it would be like Maki and Yuki minus the romantic feelings, and it would be a bit more distant, like, "Oh yeah, that's my neighbor... he's alright."
