AAIB Preliminary Report - July, 11th 2025
Crash: AI171 - Air India B788 at Ahmedabad on Jun 12th 2025, lost height shortly after takeoff, no thrust reported
Background
On 12 June 2025 AAIB was notified of an accident involving Air India's B787 aircraft bearing registration VT ANB at Ahmedabad. As per the notification, the aircraft was operating Flight Al171 from Ahmedabad to Gatwick and crashed at about 0809 UTC immediately after take- off. The notification was received from the Airport Authority of India and the Airline Operator. On receipt of the notification, a team of five officers from AAIB including DG, AAIB reached Ahmedabad on the same day. Another three officers from DGCA's Air Safety Directorate arrived from Mumbai to assist in the accident site activities and were put up at disposal of the DG, AAIB. The efforts at the site were led by the DG, AAIB and evidence collection and other site activities were carried out.
The Initial notification of the accident as per ICAO Annex 13 was sent to National Transportation Safety Board (NTSB), USA which represented the State of Design & Manufacture. As per the information notified to AAIB, the fatalities amongst passengers also included citizens from United Kingdom, Portugal and Canada. The initial notification of the accident as per ICAO Annex 13 was also sent the AAIB-UK, GPIAAF-Portugal and Transportation Safety Board (TSB)-Canada which represented the other States whose citizens suffered fatalities in the accident.
NTSB, USA appointed an Accredited Representative and Technical Advisers from Boeing, GE and the Federal Aviation Administration (FAA) to assist in this Investigation. A team led by the NTSB Accredited Representative comprising of representatives from Boeing, GE and FAA arrived at Ahmedabad on 15.06.2025 and participated in the Investigation. A team of officials from AAIB, UK also arrived at Ahmedabad and visited the site with DG, AAIB.

Progress of Investigation - The wreckage site activities including Drone photography/videography have been completed, and the wreckage has been moved to a secure area near the airport. - Both Engines were retrieved from the wreckage site and quarantined at a hangar in the airport. - Components of interest for further examinations have been identified and quarantined. - Fuel samples taken from the bowsers and tanks used to refuel the aircraft were tested at the DGCA's Lab and found satisfactory. - Very limited amount of fuel samples could be retrieved from the APU filter and Refuel/Jettison valve of left wing. The testing of these samples will be done at a suitable facility capable of carrying out the test with the limited available quantity. - The EAFR data downloaded from forward EAFR is being analyzed in detail. - The statement of the witnesses and the surviving passenger have been obtained by the Investigators. - Complete analysis of postmortem reports of the crew and the passengers is being undertaken to corroborate aeromedical findings with the engineering appreciation. - Additional details are being gathered based on the initial leads. - At this stage of investigation, there are no recommended actions to B787-8 and/or GE GEnx-1B engine operators and manufacturers. - Investigation is continuing and the investigation team will review and examine additional evidence, records and information that is being sought from the stakeholders.

NTSB Final Report - July, 10th 2025
Accident: AS1282 - Alaska Airlines B39M at Portland on Jan 5th 2024, emergency exit plug and panel separated in flight
Abstract: This report discusses the January 5, 2024, accident involving a Boeing 737-9 airplane operated by Alaska Airlines that experienced an in-flight separation of the left mid exit door (MED) plug and rapid depressurization during climb after takeoff from Portland International Airport. One flight attendant and 7 passengers received minor injuries; the captain, the first officer, 3 flight attendants, and 164 passengers were uninjured; and the airplane sustained substantial damage.
Board Member Statements:
As the findings of this report indicate, and as the probable cause suggests, Boeing Commercial Airplanes (Boeing) had no shortage of systemic issues in the leadup to Flight 1282 across numerous departments, teams, and levels of leadership. While eliminating all risk with such complex aircraft production is nearly impossible, Boeing’s failures across its training, guidance, voluntary safety management system, quality management system, and internal auditing compromised the safety of the 737 production line in Renton. Even if the removal of the left mid exit door (MED) plug in Renton had been properly documented and the removed bolts properly reinstalled on the accident airplane, I am skeptical Boeing’s systems at the time were equipped to catch these types of errors and would have successfully prevented a similar accident. To that end, we issued nine recommendations to Boeing as a result of this investigation. These recommendations include implementing a design enhancement for MED plugs, revising Business Process Instruction (BPI) “Perform Part or Assembly Removal” to ensure clarity and brevity for employees, revising training programs, and improving the safety risk management process.

Nepal Aircraft Accident Investigation Commission: Yeti Airlines 691 (YT691) ATR72-500 9N-ANC - Final Report

On Mar 28th 2024 Nepal's Aircraft Accident Investigation Commission (AAIC) released their final report via the French BEA concluding the probable causes of the crash were:
The most probable cause of the accident is determined to be the inadvertent movement of both condition levers to the feathered position in flight, which resulted in feathering of both propellers and subsequent loss of thrust, leading to an aerodynamic stall and collision with terrain.
The contributing factors to the accident are:
- High workload due to operating into a new airport with surrounding terrain and the crew missing the associated flight deck and engine indications that both propellers had been feathered.
- Human factor issues such as high workload and stress that appears to have resulted in the misidentification and selection of the propellers to the feathered position.
- The proximity of terrain requiring a tight circuit to land on runway
- This tight circuit was not the usual visual circuit pattern and contributed to the high workload. This tight pattern also meant that the approach did not meet the stabilised visual approach criteria.
- Use of visual approach circuit for RWY 12 without any evaluation, validation and resolution of its threats which were highlighted by the SRM team of CAAN and advices proposed in flight procedures design report conducted by the consultant and without the development and approval of the chart by the operator and regulator respectively.
- Lack of appropriate technical and skill based training (including simulator) to the crew and proper classroom briefings (for that flight) for the safe operation of flight at new airport for visual approach to runway 12.
- Non-compliance with SOPs, ineffective CRM and lack of sterile cockpit discipline
An Antonov An-26, operated by the Kamchatka Aviation Enterprise, struck a cliff while on approach to Palana Airport on the Kamchatka peninsu
Final Report - Interstate Aviation Committee (MAK)
The cause of the An-26B-100 RA-26085 aircraft crash was the crew's violation of the established instrument approach procedure to Palana aerodrome, which was manifested in flying with significant deviation from the set route and descent well below the established minimum descent height (MDH) under weather conditions that excluded stable visual contact with ground landmarks, leading to the collision of the aircraft with a coastal cliff in controlled flight, its destruction, and the death of the crew and passengers.
Contributing factors to the accident may have included:
- The crew's failure to execute a missed approach with the acquisition of the established minimum safety altitude (MSA) when information about the bearing indicated a significant deviation of the aircraft from the established approach procedure;
- The absence in the Palana aerodrome dispatcher's work technology of actions in the presence of information about the bearing indicating a significant deviation of the aircraft from the established approach scheme, as well as the dispatcher's passivity when such information was available;
- The lack of warning signals from the early ground proximity warning system under conditions that should have triggered it. It is not possible to determine the reason for the absence of the warning signals;
- The overestimation of the barometric altimeter readings in the final phase of the flight due to the specific airflow around the steep coastline creating a low-pressure zone and the overestimation of the variometer readings, the cause of which cannot be determined.
NOTE: This investigation report does not meet international language standards stipulated in ICAO Annex 13 - Accident and Incident Investigation. The report was published in Russian only, which required translation by 3rd parties. Due to the translation issue, the accuracy of specific details in this report cannot be guaranteed.

Aviation is so expensive, the only billionaires that don't hate spending money on it are aviators who fly and maintain their own planes. Anybody else who pays for a 13min flight in a jet, whether they own it or not, probably has a good reason for it. Jet A-1 isn't cheap, and neither are the pilots and hourly maintenance...
...Oh, and there's those pesky landing fee's, parking fee's, airport improvement fee's, ground handling fee's, air navigation service charges, and the Feds will want their cut as well.
