anon this sort of thing fascinates me. it is something I have thought a lot about and something I am happy to discuss. I also love any excuse to put together a comprehensive timeline that can be used as reference later on for fic/art/etc. purposes.
I feel like it goes without saying, but under the cut will be basically nothing but a long discussion about body weight, body shaming, fatphobia, and eating disorders, all centered around patrick. so like. maybe don't read the rest of the post if you think that's weird/it's going to upset to you. just a thought.
eating disorders in men is a concept that a lot of people have trouble wrapping their minds around. it's never really shaken off the stereotype of being a Little White Teenage Girl disease that only ever occurs because of some poor girl not making the cut for the cheerleading team. there's also something to be said about how it's incredibly uncomfortable to even broach the topic, given how people immediately go on the defensive if accused, and who are you to tell someone how to eat?
[it's also a topic that's deeply personal to me. both of my parents are/were anorexics that also dealt with binge eating [father] and purging [mother].]
patrick was pretty average-sized as a kid, slightly chubby as a tween, then average-sized again as a teenager, but he always had a babyface in the form of chubby cheeks and soft features. he wasn't ever lean like joe/pete/andy were, but he wasn't fat, either. again, perfectly average for a teenage boy, especially one that didn't play sports or do anything else particularly athletic. by 1990s/2000s male body standards, he was fine; maybe he could've been a little more "manly," but he wasn't bad to look at.
the face is one of the few places you can't really hide weight gain, especially if your face is soft to begin with. this goes double if you have to pull certain faces [wide open mouth, chin close to chest, etc.] often, which a singer has to do. imo that's why patrick grew out his sideburns in late 2004/2005; he'd started to gain a bit of weight and it made him self-conscious, especially since fob had really begun to take off around that time. seeing his face on magazines and posters and cd inserts and seeing it right next to joe/andy/pete's faces, which were much more sharp/thin, couldn't have helped. so he does what any guy ashamed of his lack of jawline does and grows his facial hair out. it creates the illusion of a jawline [creating a dividing line between face and chin/neck] even when there isn't one.

patrick gained a more weight here-and-there from 2006-2008 [gaining weight due to a breakup and subsequent admitted binge drinking/eating phase, then kept the weight on], but he reached [what appears to be] his heaviest prehiatus weight around spring of 2009, which makes sense considering the stress he was under for folie/band issues.

it seems like he decided to start losing weight shortly after that. he did have legitimate reasons to lose weight [ie, health complications], and in his defense, being 5'4" means that any weight loss is going to look more drastic on him than it would on someone of a taller stature. it also isn't impossible that he lost the weight through entirely healthy methods, though I personally doubt that.

what really gets me, though, is the fact that after he lost the weight, he started to kind of yo-yo between being rail thin and average weight for his height. it's hard to tell from just his face because again, he has a chronic babyface, but in wider shots it's easier to see what's going on. there are some photos from soul punk that legitimately unnerve me [and not just in the "concerning auras" way I joke about] that make me want to wrap him up in a blanket and make him eat a real meal.
there's a lot of theories as to why his weight dropped so quickly/why it yo-yo'd around for a bit. when asked about it, patrick tended to cite exercise and portion control, as well as cutting out certain foods altogether. that isn't inherently an unhealthy way to lose weight [and is honestly usually one of the best ways to start], but his attitude towards his weight at that time is what makes me think he might've not gone about it in the healthy way. he talked a lot about how "no one is happy fat," how he never felt attractive until he lost the weight, and combining that with the...eccentric behaviour he had during the soul punk tour means it's no hard leap to say that there might've been drugs involved. cocaine is both an appetite suppressant and something causes dysfunction in the way fat mass is stored in the body. it's also a very common drug for celebrities to use.
after the end of the hiatus, he gained a little bit in 2012/13, but generally kept the weight off for a while. that being said, the scarves started to really make an appearance in 2013, which I think is because of how he'd gone from coked-up skinny to regular guy weight, and when you're already self-conscious about it all, any weight gain [even positive gain, considering the unhealthy aspects of being underweight] is going to stand out a lot more in your mind.

he started to properly gain the weight back in late 2014/2015, and like before, the first place it began to show was in his face. there's something to be said about fathers gaining weight once kids are born [in this case, october 2014], and I think that might be partially the case for why he started gaining it back, but my other personal theory is that since he was no longer the sole face of his music, he didn't feel the same desperation to be conventionally attractive. posthiatus!patrick doesn't hide behind pete the same way prehiatus!patrick did, but it's a similar concept of being able to be one of a group rather than One and Only. that, combined with the stress of fatherhood, as well as working and touring again, is probably what made him not actively take the same measures he had in the past to avoid the weight coming back on [whatever those measures might have been].
it was also around this time that patrick started to experiment with having a beard. he didn't really commit to having a full beard, but there are a good amount of shows where he's got scruff/stubble. there's a parallel to be drawn where, just like with the sideburns, he starts experimenting with facial hair once the weight shows in his face.

it's a classic fat guy move to grow a beard, mostly because they tend to hope that hair covering the double chin/lack of jaw will hide those things' existence. to the point where I'm pretty sure every single fat guy I've ever known has had a beard at some point, if not permanently.
ever since mania, he's tended to flip-flop between light beard/real beard/no beard, but the past year or so is the first time afaik he's decided to have a grown out beard. he's also started wearing thicker jackets and flannels onstage [as opposed to the thinner cardigans he wore during mania], even during summer shows, particularly after he started gaining weight again over the past summer [2025].

there's a lot of possible causes, but the most common ones I've seen talked about tend to center on either his marriage [possible divorce], a midlife crisis [combining the beard with how he's started dressing like prehiatus!patrick in the form of the patch jackets], and/or how the weight he's put on recently [~2yrs, give or take] might be affecting how he feels about himself. there's also the chance of multiple of these affecting each other [a divorced man feeling unwanted/unattractive because of his relationship dissolving, going through midlife crisis and having issues with the way age has changed his face, wanting to look more masculine because of one of the above reasons, etc.]. it's all very fascinating to me.
I think, like you mentioned, the self portraits he's drawn over the years are the most telling thing of all in terms of how he views himself. there's a very clear difference between the 2011/2013/2015 drawings where he puts effort in to make it like, at least slightly realistic looking, vs. the 2007/2018 ones where he goes more cartoony. the 2018 one stands out to me the most because of how he drew his face as basically nothing more than an oval, and how he actually drew the double chin in.
TL;DR: patrick, at his core, has a relationship with his weight/body image that's constantly in a flux between being happy/content with it and outright trying to hide it/hating it. he tends to gain weight in times of stress [like most people], and when he gains that weight, he tends to try to hide it, especially in his face. beards/sideburns/scarves/high collars cover his jaw and neck when he's at his heaviest, with jackets/hoodies/flannels covering up his torso. all of those are choices he makes. there are occasions where he's both fat and bare-faced, but those same occasions tend to have him in tshirts, which makes me believe that on those days, he isn't feeling the same sense of self-consciousness he has for the majority of his adult life.
it makes a lot of sense that he'd be self-conscious about his weight. he's in the sort of profession where people commentate on his looks [granted, less often/outwardly accepted to do so nowadays] and the general attitude society has towards fat bodies and fat men in particular is far from kind. only time will tell if he keeps the beard permanently, if he shaves, or if anything else of note happens.
