If I have a card that's very badly damaged is it still a legal magic card? if it's so damaged even sleeves wouldn't disguise it, could I use something like a checklist card to represent it in my deck? thanks
Yes, it’s still a real Magic card.
However, if it’s badly damaged to the point where it’s impossible to not have it be marked while in your deck, then you’re out of luck and won’t be able to play the card in tournaments. (Unless the card is a double-faced card, of course, in which case you can use the appropriate checklist card in your deck.)
Checklist cards are only usable for the specific double-faced cards they list. They’re not universal stand-ins for any card you want to represent in a deck.
The only situation where you’ll be allowed to play a stand-in for a damaged card in a tournament is if that card became damaged through play at that tournament. In this case, the Head Judge is allowed to issue you a proxy to stand in for the card in your deck for that tournament only. You can’t play the proxy in other events.
If my deck has four Jace, Vryn's Prodigy and I use the checklist, how many Jace's would I need for a FMN vs a GP?
Four for any sanctioned event.
There must be a 1:1 correlation between the number of checklist cards you play in your deck and the number of double-faced cards that those checklists represent.
What are the rules for using another card to represent a two sided card?
You have to use the official checklist card that lists the DFC you want to play on it.

My opponent plays an unmarked checklist card. Obviously I immediately call a judge. How would you handle this situation at FNM vs at something like a GP?
At regular REL, we just fix the checklist. Hopefully he’s not playing other DFCs or his other checklists are marked. If they’re all blank and different cards, then we mark them, randomize them, and give him back one at random. The player needs to be more careful, but unless they were doing something purposefully nefarious, then there’s nothing to worry about here.
At competitive REL, the player will be receiving a game loss for a deck/decklist problem because the checklist card isn’t indicating any particular card. The current game ends and we fix the player’s deck again.
Can I take one of the checklist cards from original Innistrad block and write one of the new DFC cards' names on that?
No. You can only use official checklist cards that list the card you’re playing.
