Having to do something that runs the risk of hurting an injured character further, but the benefits of which or certain cost of not doing so outweigh the possible harm- the character has open wounds or fresh sutures that need to be kept clean and dry but their fever is so high the other characters are willing to risk infection by immersing them in cool water; a tourniquet will cost a character their limb if it's on for too long but the alternative is bleeding out in mere moments so their companions are in a race against time to get help; a character has broken bones which haven't been immobilized yet or they've not yet been fully examined but they're lying in the direct path of danger- flood or falling debris or fire, etc.- so their rescuers have to move them before they can safely treat them.
