One thing I need a lot more people to understand is that trans advocacy in countries with heavily gatekept trans healthcare will have very different concerns than the advocacy in countries where private, informed consent-based care covered by insurance is the norm.

Attacks on gender clinics with the informed consent model will come from reactionaries trying to get their care shut down or heavily restricted.

Attacks on gender clinics with the gatekeeping model will come from trans people demanding that the heads of these clinics stop attending conferences hosted by literal hate groups. They'll make demands like "stop sexually harassing your patients." They'll talk about how the rigid all-or-nothing model that only wants to treat "true transsexuals" is coercive, and how it can in fact make people commit to medical interventions they don't want, out of a fear that they'll be denied the ones they do want if they say otherwise.

If you don't recognize this difference in material conditions, you will inevitably end up picking fights with other trans people.
