Whenever I see some anon asking an artist if it's "okay to take inspiration" from their art it makes me want to make some kind of manifesto or billboard or PSA or skywriting campaign to undo whatever damage strange litigious illustrator types inflicted upon the most basic foundations of the creative process. i go to the shop and I ask if they have any raspberries. they say no, they used to sell raspberries, but they haven't had any in stock in the last 15 years. I ask if there's somewhere else I can go to buy raspberries. They say no, with confidence and pride, they're the only shop around who has ever sold or will ever sell raspberries. Other shops might sell other fruit, sure, but they have a monopoly on all raspberries forever. I ask if they're possibly planning on them selling them again in future? they say they can't tell me that.
on the way home, I encounter someone eating raspberries. I ask and they tell me that they grow their own, they got some seeds from the shop back in The Raspberry Days and kept them. They take me to a field of many beautiful raspberry plants and invite me to pick my own, they're free for all the town to pick whenever they'd like.

I think the word "transmedicalism" needs to be reserved for ideas that support giving authority over trans identity to medical institutions, actually.
The political project of trans self-determination can't be transmedicalist, no matter how much someone says "we need to prioritize access to medical transition," because increasing access in pretty much every real-world context is the same thing as attacking the authority of institutional gatekeepers.
Like when you're calling DIY advocates "transmeds" you have Lost The Fucking Plot. You have decided that "transmed" means "trans person whose vibes I don't like." Words actually need to mean things to be useful.
Anyone who's never had to deal with trans healthcare in the gatekeeping model especially needs to put in work to learn this: What transmedicalists are advocating for, whether they understand it or not, is a system where a cis man with a history of sexually harassing patients determines if you're "trans enough" to transition.
out trans people like to try to convince closeted/repressed trans people who are in a position to transition, to transition as soon as possible, by saying "i badly regret not starting earlier". i don't think it's a very effective argument on its own. most closeted/repressed trans people have heard the same thing numerous times, they're deathly aware of it, they're already wracked with the regret of not transitioning today, every single day. simply telling them that the regret never leaves you doesn't have any convincing power. it might only make them feel even more hopeless or guilty. to draw from my honest personal experience, i would instead tell them:
"eventually—maybe slowly, and not always all the time, but surely—you'll notice that the regret of not starting earlier is drowned out by the joy and contentness of finally feeling truer to yourself. you won't have to live stuck permanently ruminating on past what-could-have-beens anymore, you'll be able to live in the present as yourself."
